Suggest Treatment For Swelling Under The Eyes And Pedal Edema In An Asthma Patient
Question: Hello,
For around three days now a relative of mine has suffered from swollen feet (the top, toes and underfoot) and ankles. You cannot see the ankle bones due to the swelling and when she gently presses on the swollen area it dips for a few seconds. My relative has also had very swollen under eyes (worsens at night) for two months.
Her doctor took blood and urine tests which showed she has an under active thyroid but on a lower level. She was advised to take an antihistamine for her swollen under eyes incase it was allergy related. My relative's ankles swelled up around two days after taking the antihistamines (one a day) Cetirizine Hydrochloride tablets. She has since stopped taking them, as a side affect can cause swelling.
In the mornings after a night's rest the swelling in her ankles improves quite a lot, however, they swell again a short while after she gets out of bed. She spoke to her doctor again who suggests it could be the heat that is causing the feet/ankle swelling and to contact the doctor again in a week if it does not get better. My relative also has a hernia in the groin area and is asthmatic.
Please can you provide some feedback. It seems to be going on for a while especially the swelling under the eyes, and she isn't sure if it could be heart or kidney related, or whether there are allegies and heat issues going on here.

Swelling is a sign of increased fluid retention. This could be a sign of heart failure or chronic venous insufficiency. As her blood and urine lab tests are normal we can exclude kidney and liver dysfunction.
Coming to this point I recommend performing a cardiac ultrasound and checking NT pro BNP plasma levels in order to exclude heart failure.
I would recommend also performing a Doppler ultrasound of the leg vessels.

My relative had a Doppler ultrasound test on her legs some years ago and there were no problems but it would be helpful to have another one since the last one was a long time ago. Plus the other tests as you suggested.
With regard to your answer, do you also think it could be causing the swelling under my relative's eyes?
It has been over a month since she originally saw her doctor about the issue without any solution so far.
My relative is also being referred to a specialist regarding her hernia. I think they will need to do tests on her heart first if the swelling does not subside soon, what do you think?

There is no reason to repeat the Doppler ultrasound of the leg vessels considering the fact that it resulted normal before and it can not cause the eye swelling.
In the meantime I recommend taking furosemide 40mg daily in the morning to help reduce the swelling.
