Question: Dear doctor,

Around 10 days back I had pain in my left eye and I visited "Vasan Eye Care" and they diagnosed it as eye infection due to dry eye and air pollution. My left eye lid was swollen.
They prescribed Zymar ointment and Zymar eye drops along with warm water message to my eyes.

I have been using this for last 10 days approximately and it has not subsided.
I am attaching the medical prescription along with photo of my eyes (please notice swollen eye-lids of one eye).

Can you please let me know the further treatment I need to take?
What is the actual problem?

Brief Answer:
It is hordeolum of the eyelid.

Detailed Answer:
Hello
Welcome to Health care magic.

I am Dr. DADAPEER K, an Ophthalmologist and I answer health problems related to eye.

I reviewed your history and the photos attached with the question.

This shows inflammation of the glands of the eyelid in the form of hordeolum.
This is because of infection of the glands of the eyelid. The swelling is away from the eyelid margin, hence it is internal hordeolum and it is because of involvement of the meibomian glands.

The treatment is by use of antibiotic eye drops and antiinflammatory drugs.

The treatment by the use of zymar eye drops which contains antibiotic in the form of gatifloxacin and by oral anti-inflammatory drug like combiflam is in right direction.

If the swelling has not subsided by now, then we need to change the antibiotic. You can use moxifloxacin or tobramycin eye drops instead of Zymar eye drops. Along with applying hot compression provides rapid relief. Do continue the combiflam twice a day, it is required for control of inflammation.
Along with this you can consider use of oral antibiotics. I usually prefer to treat these type of infections with oral antibiotics like ciprofloxacin twice a day for 5 days.

With this treatment the swelling should subside within 5-7 days. If it does not subside then it may require incision and drainage.

Hence I advice you to continue treatment as advised by your doctor with the modifications as I have advised and do get back to me after 5-7 days.

Hope the information is helpful to you.

Thank you

Follow up: Dr. Dadapeer K (2 days later)
Dear doctor,

I have started taking combiflam and ciprofloxacin antibiotic for last 2 days.
Today evening the red swelling bump became loose and some water/blood came out.
So I pressed with tissue paper.
Is this expected?
Can I continue with your medical prescription or should I immediately visit the eye doctor?

Brief Answer:
You need to continue treatment.

Detailed Answer:
Hello
Thank you for the follow up.

Hordeolum opens itself and once it drains completely it subsides. This is the normal course and no need to worry.
You need to continue treatment and continue hot compression.
This should resolve in another 3-5 days. If itdies not resolve then you need to consult your doctor.

Hope I have answered your question and the iinformation is helpful to you.
