Question: The foreskin on my uncircumcised penis is tender and swollen , I have not had any intimate relations with anyone in months.I I am a diabetic type 2 on insulin . It looks like part is small ball of flesh red and hurting

I would keep a possibility of Candidal balanoposthitis. Classical Candidal Balanoposthitis presents with the following signs and symptoms: Itching, redness, edema, difficulty in retracting the prepuce (phimosis), white deposition and linear cracks all round on the inner surface of prepuce.
I would suggest you to use a topical Steroid+Antifungal combination cream (Clotrimazole+Beclomethasone) twice daily for 7 days.
I would also suggest an oral antifungal (either Fluconazole 150 mg single dose OR Itraconazole capsules 100mg, twice daily for a week).
An oral antihistamine e,g cetrizine once daily would provide you symptomatic relief from itching.
You should get your blood sugar levels monitored regularly because raised blood sugar predisposes to candidal infection and unless your sugar levels are well controlled you may have a recurrence.
