Brief Answer:
Phimosis

Thanks for contacting HCM

I am sorry to hear that you are having swelling of you penis. Despite being treated with antibiotics you are still getting swelling and painful urination.

From your description is sounds like you may have phimosis or a fungal infection of the skin of the penis. This usually occurs to men who are not circumcised. A fungal infection gets under the skin and causes pain swelling and sometimes difficulty urinating.

For my patients I recommend using an oral antifungal medication like diflucan.
Also keeping the penis clean by retracting the foreskin and cleaning with gentle soap.

If this is not phimosis then you may have an allergic dermatitis of the glans. For these patients I recommend a small amount of topical steroid cream like hydrocortisone to help reduce the swelling. The allergic reaction may come from the underwear fabric or the detergents and fabric softeners used in cleaning.
