But I don't think it is bronchitis or COPD.
So first of all get done PFT (Pulmonary Function Test) to rule out bronchitis.

1. Have you checked for vitamin b 12, vitamin D3 and calcium levels?

Since you have smoking history, better to first get done PFT (Pulmonary Function Test) to rule out lung related causes.
Also get done vitamin b 12, vitamin d 3 and calcium level to rule out deficiency. If deficiency then you need supplements.
