Suggest Treatment For Symptoms Of Delusional Parasitosis

Question: Hi there, my mom is 70 and she has been suffering with a life-altering issue for over 5 years. She continues to be misdiagnosed by doctors as having delusional parasitosis - since she is on a lot of medicines due to having multiple back surgeries and her age. Honestly, I thought this was what she had for years too. I thought it was all in her head. However, the last time I visited my mom - I realized she has just given up hope in finding a cure. She believes there is some type of bug in her body. What I see are open crater sores with super-tiny black dots in them - and these are all over her body. She is covered in scars and I so want to help her. Is there anything you would suggest? I do know that she visited Rotan and started having these open sores shortly after coming back from her vacation there. The sores begin as slight lumps with a yellowish color and then persist into craters - probably as a result of my mom squeezing/popping them. It's just awful and I so want to help her figure out the cause.

I should also note that she was diagnosed with lime disease year's ago. I'm not sure how many year's ago - but probably within a year or so of this skin issue running rampant.

Understood your concerns about your Mother.
Noted the history about back surgeries, lots of medicines she is taking, skin lesions for over 5 years, all over the body, since visit to Rotan. Sores beginning as slight lumps with yellowish color and then persist into craters as depicted in pictures.
My thoughts after careful study of the history and the pictures you have provided:
- The possibility of miniature insects or unknown organisms causing such a problem can be considered since these are known to occur in some patients and very difficult to pin-point the causative organism as well as to find the effective treatment hence the treatment is always on assumption basis and helped by the investigations and guidance of the Dermatologist/ Parasitologist/ Infection Disease Specialist
I would advise you the following in such a case:
Cleaning the wounds with Hydrogen peroxide and application of Liquid povidone iodine the the affected area and the skin around. This may help to reduce the infective agent load and possible curative management.
Pus/Discharge: for gram staining, AFB staining, finding the causative organisms, culture and sensitivity.
Biopsy from an active lesion.
Therapeutic trial with antibiotics, Metronidazole orally.
Multivitamins and Vitamin D and A supplements after blood tests as these are vital for integrity/strength of the skin and mucous membrane.
Consultation of a Dermatologist/ Parasitologist/ Infectious Disease Specialist may help in addition.
Control of any medical disease is also needed if present, for example diabetes or so.
Paring of nails is a simple method of control of spread.

Thank you for your feedback, Dr. Chandrakant. You noted that the possibility exists that it could be small insects or microorganisms causing this persistent problem. Are there any in particular that would present as pictured on my mom? Does this look like anything you may have seen before? My mom has been to dermatologists repeatedly - but since she was, early on, diagnosed as having delusional parasitosis - those who look into her file seem to quickly discard her concerns and send her away without taking any biopsies or questioning my mom at all. It's like she has a scarlet letter on her now. Heck, I used to think it was all in her head too - until I saw the yellowish mounds on her skin. She isn't making this up. This nightmare is very real for her. She has been prescribed multiple ointments in the past and she has also been given antibiotics - all of which have done nothing to improve her situation. So, I guess my question is, what bug-type or organism-ish thing could this be? If you have seen anything that looks like this in the past - I would really appreciate your insights.

Well, we see such patients not too often but from time to time and do treat too.
We have many from farmer community that are affected.
I myself had such an affection when we had rain-water lodged in house and street in 2006, the lesions appeared after 2 weeks of exposure exactly at the level of calf in encircling fashion. Treated with Liquid Povidone Iodine/ Betadine and Doxycyclin orally.
These are the cases wherein old, abandoned antibiotics may work.
As explained above, discharge from the lesions before they are open- that is in the yellow stage for staining, culture and sensitivity.
Biopsy from the edge of the lesion helps sometimes in diagnosis.

Part of the treatment be as already explained above.
