-Are the lesions itchy? I guess they are itchy since your treatment history mentions that you have tried montleukast+levocetrizine combination.

I have reviewed the Images.
You seem to have a condition known as Urticaria Or Hives.
Urticaria, presents as skin welts/swellings, which can be distributed all over the body. Itching is usually severe in patients of urticaria.

-I would like to know the duration since you started having it?
-Do you seem to notice any particular triggering factor (s) of urticaria in your case?

Urticaria can be acute (duration 6 weeks).

Majority of acute urticaria cases resolve within 2 weeks. Approximately half of chronic urticaria cases also resolve spontaneously within 6 months.

50%-60% of cases of Chronic urticaria are autoimmune in etiology. Association of autoimmune thyroid disease with chronic urticaria is well known.

Urticaria OR hives can occur due to various causes and types like(to name a few):

-Infections: dental, urinary tract infections, throat infections, hepatitis etc.
-Drugs: Pain killers/ NSAIDAs, antibiotics etc
-Certain foods and food additives/preservatives: peanuts, cheese, tomatoes, pineapple, fish and prawns etc.
-Physical Urticaria: Cold temperature, Heat, Pressure(Prolonged Standing, Sitting), Dermographism etc.
-Cholinergic Urticaria: in response to strong emotions, exercise, spicy food etc.
-Autoimmune disorders like SLE, RA etc. Hypothyroidism may be an association of chronic urticaria.
-Approximately half of the patients have no underlying cause and are known as idiopathic urticaria.

Treatment: An oral antihistamine like the one which you are already taking would be suitable along with topical soothing lotion like calamine would help you symptomatically. Treatment may be an extended one in urticaria patients.

You may take up a test for thyroid autoantibodies and a serum IgE blood test to different possible allergens that may be present in your environment.

Hi.

Urticaria persisting for more than 6 months is Chronic Urticaria.
As I said before, approximately half of the cases of chronic urticaria are autoimmune in etiology. This can be confirmed by an Autologus Serum Skin Test (ASST).

Since patients with autoimmune urticaria may have several circulating auotantibodies therefore in your case testing for thyroid autoantibodies is also advisable as a possible cause for underlying hypothyroidism.

Apart from thyroid autoantibodies, Hemogram with ESR, Chest X ray, Urine routine microscopy, Stool for Ova/Cyst, ANA, RF should also be done as part of routine testing for urticaria.

Also I would suggest a serum IgE blood test against different possible allergens that may be present in your environment and may be implicated as a cause of urticaria.

All these investigation might be a clue to the underlying cause.

It would be difficult to give a time limit for urticaria to go away; it may take a few days or weeks or months or even years.

Symptomatic treatment with an Oral antihistamine once daily should be continued.

A lot many cases of chronic urticaria have no underlying cause even on lab tests i.e it is idiopathic and known causes of chronic urticaria (mentioned in my previous reply) are mostly not serious in nature.
