Suggest Treatment For Syncopal Convulsion

Question: I am a 46 year old female who recently experienced a convulsive syncope after injuring my leg. I injured my quad muscle, then moved to sit down (~ 5 feet), then moved to a better positions ~5 more feet, sat down, felt dizzy and passed out. Witnesses describe that I started shaking and my eyes rolled back, and then I hit my head on the concrete and was out for about a minute, maybe less. I felt fuzzy/bad for 5 days, which I attributed to concussion. But on the 6th day I feel excessively and strangely happy. I had an "abnormal" eeg with "spikes" 2 days after the episode. I felt shaky and dizzy after the eeg, but I did not have a seizure. All other tests including blood work (at the ER), blood pressure check when changing position, EKG, and CT scan were normal. I have no prior history of feinting. I do have autoimmune hypothyroid, and I have been on a stable does of t3/t4 for years. I believe I had a concussion from the fall after feinting. Should I be worried that I feel excessively happy?

Hello,
I have gone through your question and understand your concerns.

Syncopal convulsion is related to syncope, which can occur due to postural hypotension or severe pain, unsightly environment, extreme bad odor.

Cardiac investigation should also be done.

First episode of seizure need not be treated necessarily if all investigations are normal.
Since, EEG is showing spikes then seizure activity is there and drug initiation needs to be discussed with your treating doctor or neurologist.

Drugs like Valproate, Levetiracetam, Phenytoin are available.
Increased happiness seems not to be related to the episodes.

You can upload MRI images and report.

Well, I am glad that I can just be happy. Despite how out of character it feels for the moment, I will enjoy it.

Although I cannot recall feinting in the past (except for once in high shcool after giving blood), I also recall another episode ~ 10 years ago. I fell down the stairs while carrying my infant daughter. I recall lying on the floor and making an involuntary moaning like noise. I was "somewhere else" I remember my husband "snapping me out of it" by calling my name. I do not recall losing consciousness, but it is possible. My husband did not witness the fall. I managed to land so that my daughter was on top of me and she was not injured.

Could this be related? Do I just have a strange reaction to pain/extreme stimuli?

It may be due to extreme pain or unrelated seizure episode.
You should get consultation from your treating neurologist for need of any antiepileptics.
Continue rest medications.
