Question: My husband found a lump in his right testicle and is having a lot of pain in his right side. Are these related and how should we proceed?

1. A painful lump in the right testis or scrotum can occur from acute infection called epididymo orchitis. This requires a clinical examination and confirmation by doing imaging investigations like an ultrasound scan of the testis. Other causes of acute testicular painful lump includes a inguinoscrotal hernia or inflammatory hydrocele and are less common.

2. Most acute infections can be treated with short course of antibiotics once the condition is confirmed.

3. If getting an appointment is difficult then I suggest that you take your husband to the ER immediately and have him treated.

The pain is in his rib area not in the testicles in the back by his kidney area

1. Sorry to misinterpret his pain as the area was not clearly mentioned in your earlier query. An acute infection in the testis is unlikely.

2. In the event of pain in the kidney area which has suddenly appeared along with a lump in the testis, then the first suspicion is a hernia and requires clinical evaluation.

3. There can be other conditions like a muscle spasm or kidney infection or stones and this requires detailed clinical evaluation and investigations.

4. A lump in testis at 64 years age always requires detailed investigation irrespective of whether it is painful or not.

5. A severe pain in the kidney area is an emergency and conditions involving the kidney and any bowel related condition like a hernia require immediate clinical attention.
