Suggest Treatment For Thalassemia With Abnormal RBC
Question: Hi,
I am 28 years aged male, height 5'10'' and weight is 80 Kgs.
I dont have a disease history , apart from hair thinning.
I recently underwent CBC and Ferittin Test and below were my results:
TEST NAME VALUE UNITS REFERENCE RANGE
COMPLETE HEMOGRAM
TOTAL LEUCOCYTES COUNT 6.3 X 10³ / µL 4.4 - 11.0
NEUTROPHILS 52.7 % 40-80
LYMPHOCYTE PERCENTAGE 31.2 % M:20-40; F:20-40
MONOCYTES 12.2 % 0-10
EOSINOPHILS 3.7 % 0-6
BASOPHILS 0.2 % 0-1
TOTAL RBC 6.79 X 10^6/µL Male : 4.3-5.9
Female : 3.5-5.0
HEMOGLOBIN 14.5 g/dL Male : 13.3-17.7
Female : 11.7-15.7
HEMATOCRIT 47 % Male : 39.8-52.2
Female : 34.9-46.9
MEAN CORPUSCULAR VOLUME 70 fL 76-100
MEAN CORPUSCULAR HEMOGLOBIN 21 pg 27.0-33.0
MEAN CORP.HEMO.CONC 31 % 33.4-37.0
RED CELL DISTRIBUTION WIDTH (RDW CV) 16.4 fL 11.5-14.5
PLATELET COUNT 183 X 10³ / µL 150-400
PLATELET DISTRIBUTION WIDTH 15.4 % 10.00
MEAN PLATELET VOLUME 9.5 fL 6.5-12.0
ERYTHROCYTE SEDIMENTATION RATE 2 mm / hr Male : 0-15
Female : 0-20
S. Ferittin 40ng/ml
Vid D3 45ng/ml
My Concern is my MCH, MCHC and MCV is slightly low and my S. Ferittin is 40ng/ml which is in the reference normal range (22-322 ng/ml).
Doc has suggested me iron supplements. My Mom is Anemic and has slightly low hemoglobin(around 9-10.5)
I was just concerned about Thalessemia trait as my RBC is slightly elevated and MCH, MCHC and MCV is slightly low. although in my whole family there is no history of Thalessemia ever.
I just want a second opinion on my CBC results and my concern

Brief Answer:
Iron deficiency anaemia
Detailed Answer:
Hello XXXXXXX
You just have iron deficiency anaemai.
Your reports typically suggest that.
Thalassemia presents with abnormal RBC.
If you are in doubt of thalassemia, please get a peripheral smear done.
Thalassemia can be diagnosed only with presence of abnormal red cells in peripheral smear.
Let me know if you have any other doubts.
Thank you

Is 40ng/ml Iron content is deficient enough to lower down MCHC, MCH, MVC?
Is RBC Count in my report abnormal enough to suggest thalassemia or should I continue with the Iron Suuplement and get a CBC done after sometime ?

Brief Answer:
Reports are normal
Detailed Answer:
Hello
40 ng/ml is not very low.
It is fine above 20.
You have a very mild anaemeia.
The reports dont suggest anything towards thalassemia.
Please dont worry.
Even then if you have doubts please get a peripheral smear of blood done.
It is the most important and easiest test to diagnose thalassemia.
Thank you
