Suggest Treatment For Throat Infection And Prostate Infection

Question: hi my eosinophil count is 25%. I have been suffering from throat infection for the last 8 days, also i am taking antibiotics for prostate infection. I am allergic to poultry and dust. Is this level of eosinophil count indicate something serious.

Sir,
Thank you for the prompt reply. Answers to the questions which you asked are as follows:1. Throat problem relieved but since january this was the third time i suffered throat problem with difficulty swallowing. 2. Allergies affected me since last 4-5 years but not serious one like sneezing etc. 3. Nose symptoms-congestion. 4. Headache-mild over eyes for the last 3-4 days. 5. suffered low grade fever during throat infection with body ache. 6. Difficulty breathing or say not so comfortable breathing for the last 4-5 days which gets a slight relief since i started taking heterazan 100mg. 7. I am taking Hetrazan 100mg, montelukast and allegra 180mg. 8. I have never been investigated for allergies this is based on my guess which i arrived at by exclusion method. 9. Previous eosinophil count 2% (20-Oct-14).
Please guide me about the possible causes and treatment options.
Additionally, i would like to tell that I am experiencing decline in stamina, breathlessness on physical activities like climbing stairs (for the last 9-10) days and body ache.

Hello,

Thank you for your follow-up.

I would like to make suggestions for you as follows:

1. As you have possibility of respiratory allergies, I would suggest you allergy testing for common aeroallergens like house dust mites, molds, pollens, insect danders, animal proteins (if you have contact with domestic animals), etc.

This will help you identify the substances causing allergies to you as well as to know the measures to avoid them.

2. Based on allergy testing, an Allergist-Immunologist may prescribe you allergen specific immunotherapy which works on immune system to gradually improve allergy symptoms.

3. I would suggest you to continue montelukasst, allegra & Hetrazan.

4. I would suggest you to repeat eosinophil counts approximately 4-6 weeks after completion of Hetrazan course.

5. As you are experiencing decline in stamina, exertional breathlessness and bodyache, I would also suggest you to check hemoglobin and treat anemia if hemoglobin is low.

6. Regular warm saline gargles and steam inhalation will also help you to recover earlier.

7. Please avoid exposure to dusts, smokes and air pollution as much as possible.

8. I usually suggest my such patients a good multivitamin-multimineral supplement after recovery.

Thank you once again sir. This is the first time i am using this platform and find it quite helpful. I am satisfied with the help extended and have a couple of questions;
1. Is Eosinophil level of 25% indicative of malignancy or is it usual to have this level of eosinophil.
2. You advised to check eosinophil level after 4-6 weeks of completing Hetrazan course. Why not just after completing the course of after 10 days of medication. How much it should come down during the first week of medication.

Thank you for your feedback and for appreciation. I am happy that my answers were helpful to you.

I would like to reply to both of your questions as follows:

1. There are numerous causes of Eosinophilia. Common causes are allergies and worm infestations. Rare causes are numerous like effects of some drugs, some rare infections, some rare malignancies, etc.

Eosinophil percentage themselves suggests nothing except the fact that they give a clue towards diagnosis. It is the symptoms and physical examination of the patient that is more important for diagnosis.

If you have symptoms like weight loss, loss of appetite, excessive sweating, etc, then only I would think of some serious condition like malignancy, tuberculosis, etc. Of course, your physical examination is best done by your regular doctor.

Hence, please do not think of malignancies on base of any report unless your treating doctor thinks the possibility of malignancy.

25% eosinophils are not "usual", so it is important to identify the cause.

2. Hetrazan is effective for eosinophilia caused by some parasitic infections. So it will bring down the eosinophils if eosinophils were high due to those kind of infections. It will not normalize eosinophils if there is some another cause like "allergy".

Please note that allergy is abnormal functioning of immune system, it is best diagnosed by detailed history of symptoms & allergy skin testing. Personal experience of symptoms triggered by some substances does not mean that you are really "allergic" (immunologically) to that particular substance.

I have advised repeat check of eosinophils after around a month of completion of course of Hetrazan because eosinophils are generated in bonemarrow, enter circulation and then persist in circulation, the whole process may take a few days. So it is important to have a lag time period before repeat testing.

Second, there is no treatment of "high eosinophils", treatment of the cause causing high eosinophils is only available. Once the cause of eosinophilia is identified and treated, there is no meaning of "checking" whether they became normal, decreased or increased (unless for some rare diseases like malignancies).

If you have high eosinophils even after 1-2 months, allergy evaluation and treatment of allergies would be important. If allergy evaluation is negative, other causes of high eosinophils would be thought of.
