Question: I've been having an occasional throbbing pain in my left mid to upper back on left side which has become somewhat chronic during the past week.

Thanks for your query, albeit short and specific.
To recapitulate: Male/64 - occasional throbbing pain - left mid to upper back - chronic during the past week - tried OTC pain and anti-inflammatory medication ...

Occasional throbbing pain can be due to the following reasons:
- Muscular spasms.
- Go for EKG and cardiac evaluation to rule out any heart related issues as the pain is on the left side
- May not be infection causing inflammation as it can not be intermittent as you have pointed out about being occasional. The pain of infection is continuous and gradually increasing and must have been an abscess by this time.
- Inter-vertebral joints inflammation may cause similar pains.

I would advise you the following in such a situation:
EKG and cardiac evaluation as mentioned above.
Local examination for mass, muscle spasm knots, local tenderness and so on.
If needed an X-ray of the chest and the back
Getting a correct diagnosis and proper care/management.
