Suggest Treatment For Tinea Cruris On Belly Inspite Of Using Fluconazole

Question: hi I am 30 yr old male hairy and I sweat alot. I had tinea curitis on my arm and belly. I took one FCN every week for 2 weeks with candid b cream application. the arm rash is gone but the one on belly is just the same. infact now there are lot of red spots in vicnity also. what is this pls help
2) I dust my groin and pubic area with clotrimed powder. however the groin itches alot there is sharp itching on testicles and groin both despite using cream and powder. kindly advise. on itching sometines the skin breaks and it looks like mosquito bites I can feel it bumpy also on touch .

I have seen the attached pic also. Thanks for attaching the same.

You said you have taken tab fluconazole and applied candid b cream.
I will like to mention you that :
1) candid B Is a combination of antifungal and steroid. As a dermatologist, I dont prefer to give this combination. It is usually associated with early relief but along with early reappearance of the lesions also. Sometimes it results in modified tinea which takes longer to get treated.
2) in my experience, I have seen resistance against fluconazole. So despite appropriate treatment, patient doesn't get improve.

I usually advice my such patients to take tab terbinafine daily dosage and apply antifungals like miconazole / sertraconazole cream twice daily. You may continue applying clotrimazole powder. You should discuss about these with your treating doctor.

I will also like to know about your blood sugar levels.

I got my blood sugar tested last month it was fine.. also I had used sertraconozole for a week prior to using candid b. you are corrrct I got better results with candid b . however my abdomen area rash. how long should I take these tablets? trust you have had s chance to go thru the groin irritation area pics also. kindly advise

You should take these medications for atleast 2 weeks.
I saw your pics. But couldn't see the groin area pics. Kindly upload the same.

I will stress upon discontinuing candid b cream that is steroidal combination once again.

You may continue the treatment under supervision. For symptomatic relief, I will add levocetrizine or fexofenadine night time. Keep the area clean and dry.

hi thanks the last two pics in the attachment are groin /thigh. pls see and advise if something else needs to br done as the rashes on abdome and on thigh/ groin are different. thnks.
also pls advise would you advise applying terbinafine cream or powder instead of the tablet. i am normally wary of taking any medicine for so long. laso once you see the pics of groin/thigh can you pls advise if different treatment will be required since the rashes are different and there is no itching at all on abdomen area.

I have seen all the attached pics.
it seems to be same .. fungal infection only.

You may apply terbinafine cream also. But I have seen usually patients do not respond to tab fluconazole.
Alongwith apply clotrimazole powder in folds.
For itch, you make take antihistaminics as I already mentioned in my previous replies.
Wear cotton clothes.
