Suggest Treatment For Tinnitus And Vertigo

About 6 years back in 2011 , when i stretch my body in lethary in the early morning , i bent my head towards leftside in an awkward position. At the very moment i heard a click like sound in the mastoid bone area behind the ear lobe and immediately when i got up for the first time in my life i had spinning around me. So i went to my ortho for treatment and also for my cervical spondlosys which was there since 20 years.Apart from this i had Tinnitus, ringing in ears, vague sound in my left ear with 60% hearing loss in left ear and 40 % loss in right ear.With all this i occassionally get the vertigo..So my ortho refered me to an ENT saying i have BPPV and also asked me to take an mri of cervical spine, as i have spondylosis problem since a long time , which i took it. There in the MRI by chance found a small rice grain size cyst in the inner wall of the spinal canal at c4 and c5 vertebrae.

My ortho asked me to wait and watch besides taking medicines for cervical spondylosis and ear problem, which i followed. This was happend six years ago. Now Iam 75 year old. I have occassional vertigo and no other symptoms all these years .I ) get vertigo when i suddenly get up from the bed, 2).when i moved my head back3). when i moved my head forward or 4) when i sleep on the bad side that is left ear down and some times beep sound and sometimes i feel as if some balls are rolling in my left ear.. if i get up from the bed after turning my whole body to left side , i do not experience vertigo symptoms. For the past 6 years i am sleeping on the left side of my body keeping my left ear upwards. I never had any discharge from either of the ears. Besides BPPV , i have Diabetes, BP , BPH and hypothyroidsm.I am not obese and i am lean .65. kgs.All the health conditions are well under control. I take Vertin 16 mg (SOS) twice a day for .

I request you to give me your opinion about the cyst and this vertigo problem.Do i have to live with this vertigo forever. Or Are there any effective medicines now available in India? Do i have to go for surgery for my ear? Or do i have to just avoid it?

I evaluated your query thoroughly.

*Regarding tiny cyst in spinal canal at C4 & C5 vertebrae, surgery is not advisable
& not mandatory considering your age and all other narrations.

*Regarding vertigo if you are satisfied with your current status with vertin it`s
fine else detailed Neurological evaluation clinically with Audiometry as well as
MRI Brain clears further ways of management.

*Effective medicines are available every corner of the world but the thing is
diagnosis of the lesion then comes the definitive management aspects.

*Surgery for ear also in my opinion would not be rewarding to that extent .

Dear Doctor Namaste. , Thank you very much for your advise. Exactly i too am thinking on the same lines as what you have advised.Please advise me shall i take stugeron 25 mg along with Vertin 16 mg whenver there is an episode of vertigo? If so for how many days for each vertigo episode? is it necessary to go to audiometry, since i have undergone in 2014 and 2016 and recently .Everytimme, the hearing loss is 60 to 65% in left ear and 40 to 45% in right ear.ON the other hand if i use a hearing aid in my left ear, does it help in any way to subside vertigo?

*It`s fine you can definately take Stugeron 25 and Vertin 16 at the time of episode
of vertigo usually both tab. once daily for 2-3 days only not more, if vertigo
persist then can take further but not necessary once episode is fairly subsided.
*If Audiometry in 2016 , not necessary at this juncture.
*Any hearing aid in any ear is not going to anyway subsiding vertigo it`s out of
question.

I will frankly opiny not to undergo any further surgery or investigation at all
just make your lifestyle adjusted with current situation with taking care of
rest of the things & live happily.
