Suggest Treatment For Transverse Myelitus

Question: My Mother is suffering from Transverse Myelitus from Dec 24 of 2009, she is in XXXXXXX and the treatments are not doing any good. Is there any treatment that can be given to her?

Hi, I have gone through your question and understand your concerns.

Patients with Transverse Myelitis recover partially mostly during first year of disease, this is strongly related to cause of Transverse Myelitis ( e.g. autoimmune, viral) and severity of symptoms.

Corticosteroids are among medications used in first stages of the disease.

When no partial recovery is seen, treatment consists in improving symptoms and psychological state.

If Gabapentin isn't working for the pain, Pregabalin ( Lyrica) should be tried.

Baclofen or Diasepam could be used for muscles stiffness.

Physical therapy also applied regularly improves muscle spasms and pain.

You should discuss with your mother's primary care Doctor above mentioned treatment possibilities and dosages.

My mom is taking Gabapentin-900mg per day and baclofen for spasm - 5mg and also taking Multi vitamin. She has sensation all over the place below waist line. She has her inflamation on T9. She sets up the caliper and walks with walker for one hour a day but if you remove the caliper she will loose control of hip. Currently she doesn't have control over hip. Every morning she stands by holding on the window rail for about 20 mins. She never had BP in 2012 doctor advised to take olsar H tablet- 10mg and also for cholestrol she is taking atrovastat - 5 mg, this also started after she was diagnosed with TM. but its under control.

She does lot of physiotherapy, exercise with small dumbells for hand and she does kick the ball by sitting on the bed by hanging her leg down. Now when we touch her on any area below waist lin she can sense it. She is doing much much better compared to the first year but still not able to walk by herself. Is there ay other thing that we can do, or any treatments that Doctor can suggest in XXXXXXX XXXXXXX She is residing in XXXXXXX XXXXXXX She is a nurse by profession so knows ll the medical conditions. Is there any exercise that can help to get the hip balance as we try to change her exercise routine quite often.
We asked our Doctor whether we can take an MRI to see the progress and he mentioned no need for an MRI until we see a good change.
Mom also took Corticosteroids for first 4 months and also did gamma (IVIT) initially.
Also mom has a strong will power. does cell therapy help in any ways.

So there was some progress about your mother's condition, and this is a good thing.

A spinal MRI is necessary at this moment to evaluate progression by imaging study too.

Your mother's movements are not sufficient to strengthen the hip and legs muscles, there is need for continuous physical therapy with appropriate exercises ( pool exercises e.g.) to strengthen muscles and regain balance.

Cell therapy ( I assume you mean staminal cell therapy) is still in its experimental phase.

Doctor is there any doctor you can suggest so my Mom can see, she resides in Puthoor, Kottarakara, XXXXXXX XXXXXXX (South part of India).

Also is there a list of exercise with diagrams so my Mom can check whether she is doing correctly. If you can refer a physiotherapist who is local is definitely appreciated, We did have one but that person charged us on an hourly basis and we ended up paying huge amount. Let me know, thanks!
XXXXXXX XXXXXXX university is researching on TM is there any findings on the medicine yet?

Those reports ( I read them finally) are from 2010, so from what you prescribe about your mother's actual condition, seems to me a significant improvement, so a MRI could help evaluate better the degree of progression.

Sorry I can't help suggesting a Doctor or Physical therapist in XXXXXXX because I'm not resident in XXXXXXX I work and live in Europe.

Studies are underway not just in Johns XXXXXXX they are directed towards humanized antibodies, but there is no a definite result yet.

In my opinion, your mother should continue physical therapy in a specialized center for this purpose.

Is there any doctor from South of XXXXXXX who can refer a neurologist and take a second opinion as Mom's current doctor told thee is no need to expose to rays by taking MRI as of now. Kindly advice, thanks a lot!

MRI imaging study doesn't use rays, there are no damages or negative effects on body, so there is no contraindication about MRI, except cases with metallic parts implanted in body tissues.

Your mother can have MRI imaging studies as needed with no negative effects.

You can ask for a second opinion, it is your right.
