Suggest Treatment For Tremors, Lack Of Concentration, Erectile Dysfunction And Anxiety Disorder
Question: Hello doctor, lately since I've been back at school, i feel disconnected. It is my third year, im just 19, and i just don't feel myself. I don't have the same personality i feel like. I notice my hands has been shaking a lot lately, its harder to focus, I'm not as quick witted. I look at myself and i just don't like the person i am. And when it comes to sex, I've been unable to get an erection multiple times and its ruining my relationship with a girl that is the only thing that makes me happy. Which has to be the most embarrassing thing that has ever happened to me. I'm only 19, that shouldn't even be a thing. She makes me happy and going out with friends, alcohol just kinda helps to make me forget about everything. I don't know whats wrong with me doctor, but I'm scared and on top of that I'm to scared to go talk to anyone about this. This is the first time I've said anything about this to anyone. I don't know, I just felt this would help.

What other details would you need?
Brief Answer:
Details needed below
Detailed Answer:
Hello,
Thank you for your follow up question.
--Please let me know, how often do you have this hand shaking? - let's say per month.
-- How long does it last?
--Except hand shaking, what other symptoms do you have? - shortness of breath? Tightness in the chest? tingling? etc...
-- How is you sleep? Your appetite?
-- Do you take any medication for other problems?
If these episodes happen often enough to not allow you to live a usual daily life, then I think that Alprazolam only in crises wont be enough and I will suggest to take a medicine regularly.
Also, one thing which will be very helpful, it is to go to a specialist (Psychiatrist) who provides counseling. Is there any chance for you to consider it?
Looking forward to your follow up question.

The hands are constantly shaking it feels like. I'll take moments to stop and look at them and they just can't stay still. It lasts until I stop paying attention and get my mind on something else so I don't think about it, even though I know it keeps happening.
I stumble over my words a lot, it's harder to talk, which is weird. I just have like a weird feeling in my chest area all the time, I don't know if it's tightness or what it is.
My sleep is all over the place, I'm always tired and when I try to sleep, I can't. My appetite is weak, I eat once a day, maybe twice. But that somehow seems to be enough.
No I don't take any medication.
I don't know about a psychiatrist. That just sounds like a lot & to be honest terrifies me.

Brief Answer:
Suggested medication and other practical advises that may help
Detailed Answer:
Hello dear,
It clearly seems that you need to take a regular treatment and get out of this situation which it is clearly affecting your daily life.
What I recommend in this case it is Sertraline 25 mg once/day (in the morning) the first week then continuing with 50mg once/day. It will need at least 10 days to start giving effect and it may make you a bit irritated. So in this cases, use Alprazolam 1mg as needed, XXXXXXX 3 times/day, but not for longer then 3 weeks.
Also, consider going to the gym because physical activity will help a lot.
Don't take alcohol more then 14 drinks/week (2 per day max), avoid coca-cola, coffee, f possible...so basically avoid things that are stimulants.
Talk with someone, for example your girlfriend. Keeping all this feelings for yourself, will make it harder to get better and also you don't have to keep all this burden yourself- people who love you are there to support you.
A counselor it is not a psychiatrist, but it is a specialized person with whom you can talk easily and who can help redirect your thought int addressing the negative/non healthy ones, and they can suggests tasks that can help you work with yourself. People choose to see a counselor sometimes even if they they just need an advice when they are not able to take a decision, or even if they need to talk with someone. I would suggest to consider it, but then it is u to you.
Hope this is somehow helpful. You may need a prescription for the medications recommended that you will need to take to your local doctor.
Please feel free to come back to me if you have any more question or concern.
