Based on your query, my opinion is as follows:
1. Blood sugar is under bad control.
2. Lipid profile abnormal, with mild lipase elevation.
3. Mild iron deficiency is also seen. Mild reduction in cellular hemoglobin levels, overall good.
4. Increased uric acid is possibly due to protein rich diet

You will need
A. Lifestyle changes - weight reduction, moderate exercises and most importantly diet control.
B. Oral hypoglycemic drugs dosage needs increase, however, injectable insulin can be tried.
C. Iron supplements are not necessary at present.

Hi Dr,I forgot to mention that I am also taking Lantus Insulin 24 units 9.00 am once a day. Basicaly I am a vegetarian. What lifestyle changes do I need as mentioned by you.

Thanks for asking again.

If you are already on lantus insulin, you might need to take it twice daily, rather than once. The poor control, as shown by HbA1C is rather serious and at your age, future risk of cardiovascular events, neuropathy and vasculopathy are serious.
The dose needs to adjusted along with your diet.

For your age, lifestyle modifications is much easier to apply for better control of diabetes mellitus, hypertension and abnormal lipid profile. You can try:

a. At least 30-45 minutes every day, moderate physical activity like brisk walking, cycling or water aerobics.
b. Body weight reduction to maintain normal body mass index.
c. Changing refined carbohydrates with whole grain and increase in dietary fibre.
d. Reducing amount of saturated fat in diet.
e. avoid smoking and alcohol
f. avoid sugar sweetened beverages
g. Make every meal balanced and equally nutritious and calories too
h. Stay well hydrated and take medications regularly. Check blood sugar and HbA1C at regular intervals.
