Suggest Treatment For Unusual Rise In Blood Pressure

Question: Hi
I am 35 years old, i have 4.5 mm stone (as diagnosed 6 months back) in my left upper kidney and also stones in my gall bladder. My blood pressure is high for last two weeks (140+/90+). i do daily 30 to 60 minutes walk, drink plenty of water (at least 2 ltrs a day), intake of salt is near to nil, never eat fried, fatty food , daily do breathing yoga. drink alcohol in moderation (maximum 2 drinks a day). but my BP is not under control. i got viral infection also these days (cold and soar throat).
why my BP is rising? how can i control it?

Welcome to health care magic,

I have read all your queries carefully and will answer it one by one.

1) 4.5 mm renal stone could be removed while passing urine for which you will need to take plenty of water orally (4 to 5 liters per day) with tamsulosin (tab.veltam 0.4 mg) daily once till the next follow-up to your treating doctor.

2) For gall bladder stone,you will have to consult general surgeon whether to go with conservative management or gall bladder removal operation as per your clinical features and laboratory investigations.

3) In spite of taking all the precautions,if still your blood pressure reading shows systolic 140+ and diastolic 90+ then you should start antihypertensive medication.

There can be many factors like stress,hereditary,stiffness of arteries,high lipid diet which could rise your blood pressure.

If you would be my patient,then I will start antihypertensive with calcium channel blocker(amlodipine 5 mg) with salt restricted diet and life style modifications.ACE inhibitors like enalapril is also a good first choice drug.

Although, antihypertensive drugs may vary according to physician's choice.

You should undergo routine hemogram,liver function tests,creatinine, lipid profile and 2D echo cardiography to check the health status of heart.

You treating physician will know about the investigations and write accordingly.

4) your viral throat infection and cold is not a big issue but you can take symptommatic over the counter treatment like antiinflammatories, antihistamines and antibiotics (if required then consult your GP) with steam inhalation 2 to 3 times a day.

Hi Dr.

Thanks for your consultation.
Just want to ask if I start taking medicines then will i have to take these life long or just for a time being to control and manage the BP?

Thanks for immediate follow-up question.

In most cases of hypertension, medicines required life long and to be taken even after Blood pressure is well maintained because it might cause rebound increase in the blood pressure on stoppage of the drugs.

In some cases what I have seen in my clinical practice so far,if patient takes this regularly with all the necessary precautions including life style modifications then his antihypertensive can be withdrawn (when there is no hereditary history of hypertension) but it should only be done with your physician's advice and heart investigations reports.

In your case,I would advice you to start your antihypertensive at earliest after the consultation to a local physician and you should check your blood pressure regularly with timely follow-up.
