Question: My daughter is a type 1 diabetic, takes insulin 4 times a day, early Saturday morning went to ER at local hospital due to having high fever, uncontrollable shaking, was diagnosed with kidney infection , given pain medication with Bactrim two times daily. When her fever goes down she starts shaking and gets real pale, then her fever goes back up, is this normal for kidney infection, should she go back to the ER for further evaluation.

Brief Answer:
Yes, It will take some time to subside.

Detailed Answer:
Hello,
Welcome to Healthcare Magic,

The history and symptoms of your daughter suggests me that she is having some infection in the urinary tract and your doctor has diagnosed it as a kidney infection or pyelonephritis.

As she is a known case of Type 1 diabetes mellitus which also increases the chances of developing urinary tract infection (UTI). As your doctor has prescribed antibiotic BACTRIM (sulfamethoxazole and trimethoprim) which is an excellent medication against the kidney infection and it may take 1 week or longer for symptoms to go away completely.

Finish all of the antibiotics, even if she feel better. If she do not finish the whole dose of medicine, the infection may return and be harder to treat later.
You may need to go to the hospital if she is very sick and cannot take medicines by mouth.

I would suggest her to undergo blood tests such as CBC and blood culture. she may also need the following tests to rule out other problems in her urinary system like CT scan of the abdomen, Intravenous pyelogram (IVP), Kidney scan and Voiding cystourethrogram.

I wish the symptoms will subside as early as possible. If you have any query feel free to post them.
Thank you.
