Considering that your sister is aged 45years, i am sure fertility would not be a concern at present and if she is asymptomatic ( as in no increased pain during menses or dysmenorrhoea or menorrhagia-excessive menses with clots causing anaemia) then they need not be removed at all.Considering these are benign lesions, one may even live u with them.
The treatment is offered for symptomatic fibroids which cause pressure symptoms like increased frequency of urine or chronic pelvic pain as in cases of subserous fibroids or intramural / subserous fibroids causing heavy menstrual bleeding and pain.
If she is not keen on surgery then she may consider the use of a drug ULIPRISTAL 5mg daily for at least 13 weeks which can help reduce the symptoms and also decrease the size or she can be placed on GnRH analogues like INJ. ZOLADEX or leuprolide which can also help decreasing size of uterus and can provide symptomatic relief. The need for surgery can be decided later if the medical management fails. You may discuss the options with your gynecologist and take an informed decision,
