Suggest Treatment For Vaginal Fissures

Question: I have vaginal fissures and my doctor prescribed an ointment normally used for anal fissures, it contains Nitroglycerin 0.2% in Lidamantle HC Diltiazem
2%/Hydrocortisone 1%/Lidocaine 2% cream. I was not given any documents with instructions nor warnings. when i looked up the medication on the internet is says not to apply to vagina nor to use if you are on blood pressure medications. I am on losartan 100mg, my doctor knows this but i am worried if this ointment is safe to use. I do have a family history of heart disease but I have not had any heart or stroke issues other than high blood pressure controlled with medication, cholesterol is borderline high 202, overweight 166 @ 5'1". Should i take this medication? Do you have any suggestions on what else i should try to heal these fissures? I have had them for two months now, very painful.

This ointment is not harmful.. It can be applied over the fissure area in the vagina. Avoid intercourse till fissure heals completely.. Local application cant increase the blood pressure all of sudden..

Has these fissures appeared all of a sudden or there was any triggering factor like masturbation or delivery? You can apply plain lignocaine jelly and hydrocortisone cream alternatively to heal the fissure.. If it doesnt heal within 2 weeks time then apply the cream which your doctor has provided.. Warm saline sitz bath should be taken twice daily to reduce pain..

It started with an itching feeling and I may have scratch the area which possibly contributed to the original fissures. Last week an additional fissure occurred during intercourse in the perineum region, felt like a razor slicing my perineum. It has also been very dry, no discharge so I don't think it is a yeast infection and my doctor dismissed the idea when suggested by an assistant. I plan on switch doctors since she has not been very informative when providing medication nor does she seem to be a caring doctor. She did not give me instructions or warnings for the Premarin she prescribed (like avoid using during sexual intercourse) nor for this additional ointment. I've had to resort to the internet to get the information I need. A year ago she switch me to a low dose birth control pill and I am wondering if this may have contributed to the weakening of the lining? The last time they checked I was a long way from menopause, that was about a year ago.

Dont worry.. Dryness along with fungal infection can be a contributing factor for fissure..

Get a vaginal swab to rule out infection.. If there is infection, get it treated first.. Use lignocaine and hydrocortisone alternatively to treat the fissure.. Avoid intercourse till fissure heals completely.. If there is persisting dryness, use lubricant prior to intercourse.. Treat the partner also if there is a infection..
