From your history, I presume you are experiencing primary vaginismus, or the condition where consummation of marriage has never happened.
To manage the situation, evaluation of the problem has to be done in detail. The several causes include pelvic infections, urinary infections, severe stress, childhood or adolescent anxieties, high ethical values which consider intercourse as immoral, and so on.
To understand the difficulty and allow to grade the condition, it is necessary that the attending doctor assesses all these factors step by step. The tightness of the pelvic floor muscles, which go into a spasm on introduction of any foreign body into the vagina, is also assessed, along with the general reaction of the person.
This condition is highly treatable. After excluding/treating infections, psychological counselling is given along with pelvic floor training by experts. Slow dilation is given to allow relaxation of the pelvic floor. Couple counselling is also given to enhance relaxation. Medications like Botox have been shown to help in pelvic floor relaxation. Anxiolytics can also be tried.
