Question: question on 87-year-old man with one kidney (other removed for cancer years ago), with heart problems (had valve replaced 2 years ago), in hospital with creatinine level of 4 yesterday and 3 today. What does this mean? Dialysis? Thank you!

Thanks for your query.

As you have single kidney, you are vulnerable to any kind of kidney injury how ever trivial it may be. Can you please clarify some of following queries?-

1. do you have diabetes and hypertension
2. when did you get your creatinine test done last and what was the report?
3. we're you told to have kidney disease or high / abnormal creatinine at the time if valve replacement surgery or kidney removal for cancer?what was the creatinine that time?
4. are you taking any treatment with any kind of drugs like pain killers, antibiotics and cancer therapy drugs?
5. why did you undergo the test this time? do you have any complaint / problem or a routine rest done?
6. what is your proteinuria amount in urine?

1. no diabetes but possibly hypertension/heart disease
2. creatinine test yesterday was 4. Today was 3.
3. no to kidney disease. Not sure about the cre. test then.
4. no pain killers, cancer drugs or antibiotics. Am sure he is on many other drugs (this is a friend's father so I am not sure)
5. was not feeling well so went to the doctor.
6. no idea

Brief Answer:
Need evaluation.

Detailed Answer:
As there is a drop in creatinine, he must be having an acute kidney injury and another possibility is acute on Chronic kidney disease. He is likely to improve further and may be get stuck at creatinine of around 2 mg/dl. We need to follow him closely and carefully. He also must be evaluated for urine culture and serum protein electrophoresis. We need to check his calcium, phosphorus along with Vitamin D and PTH hormone assay for his general complaints. Kindly discuss about this your treating doctor.
