Suggest Treatment For Varicocele
Question: My cousin (35) and his wife had trouble to conceive for about two years. She was pregnant twice but had miscarriage around week 6-8 both times. They has been trying to conceive again for a year now (she has been doing period monitoring in a fertility clinic each month) but nothing has happened. Recently my cousin was found to have varicocele from an ultrasound, and his sperm DNA fragmentation is high (68%). They live in China and some doctors in private clinic recommended surgery and others in public hospital clinic didn't think this is a big deal. Some fertility clinics don't like to accept patience with high DNA fragmentation for IVF procedure. They haven't tried IVF yet but they are confused on what to do next. So they hope i can help them to consult doctors outside of China. Any recommendations would be much appreciated.

Following is my reply:
1) Varicocele surgery is not associated with improvement of semen parameters or DNA fragmentation index (DFI).
2) I don't advice surgery for varicocele.
3) Most IVF centres in XXXXXXX do accept patients for IVF though DFI is high as patients with high DFI do conceive.
4) They can try for IVF in XXXXXXX

Hi,
Some degree of varicocele is normal in many men. Severe weakness in wall of veins in scrotum causes varicocele. There is no fault of the male in this and no history of family required.
