Suggest Treatment For Vertigo
Question: I am a 63 yr. old woman. 17 months ago I suffered a sever Vertigo attack. I am still suffering with light headedness. I went to an ENT and had several procedures , turbinate reduction, and an ear tube. Still I feel a fullness in that ear, and am light headed. They said I had euestation tube dysfunction. Is there anything I can do for that

1. It is important to list all the details of the vertigo and check whether the following common causes were missed:
a. Benign Paroxysmal Positional Vertigo (BPPV)
b. Meniere's disease
2. BPPV is a positional vertigo.
Meniere's disease is diagnosed on the following criteria:
a. Vertigo
b. Fluctuating hearing loss
c. Tinnitus
d. Fullness in the ear
There is usually nausea and vomiting, the hearing usually worsens during an attack and the tinnitus also worsens.
2. You should get a PTA (Pure Tone Audiogram) to document any hearing loss. Meniere's disease usually causes low frequency hearing loss initially (Audiogram sloping to left).
3. A Tympanogram is not possible as you already have an ear tube. However, steroid drops can be given through this ventilation tube directly into the middle ear.
4. An MRI Scan is usually advised in a patient of vertigo so as not to miss any major cause.
5. BPPV is treated with re-positioning maneuvers. Meniere's disease should normally respond to a salt restricted diet, acetazolamide (diamox) and betahistine (Vertin).
