Suggest Treatment For Vitiligo
Question: Being of XXXXXXX decent, my natural skin color is brown. However, I have a small area with some discoloration & my skin is white (or a much lighter shade of brown) on my foreskin of my penis. I was wondering if that should be of any concern.

I have attached the photos. In two photos you can see white on the foreskin. In another photo the tip appears of a different color than the rest of the penis. Thank you.
Brief Answer:
Vitiligo or lichen sclerosus et atrophicus, answer few questions
Detailed Answer:
I have reviewed the attached photograph and the tip appears fine.
I could appreciate depigmentation on the foreskin.
There are 2 possibilities: either vitiligo or lichen sclerosus et atrophicus.
I would like to ask you few questions:
Are you having any change of texture over the affected skin?
Are you having itching, burning or any other symptoms on the affected area?
Are you having depigmented areas on other areas of the body?
Do you have family or personal history of vitiligo?
What is the duration of the colour change?
Kindly revert with the answers.

Brief Answer:
Seems to be vitiligo, topical steroid & tacrolimus ointment
Detailed Answer:
According to your complaints it seems to be vitiligo most likely.
If I was your treating doctor I would recommend you to apply a topical steroid like clobetasol propionate cream on the white spot in the morning and tacrolimus 0.1 percent ointment on the affected area at night.
These are prescription drugs and you need to see the doctor in person to get them.

1. Dr thank you for the recomendation-
2. So you recommend clobetasol for the vitiligo on my penis? What are the side effects of this medication? What happens if I don't apply clobetasol to the affected area on my body?
2. A few weeks ago a dermatologist diagnosed me with lichen on my leg & butt (which has been extremely itchy for the last year or more) - he also recommended that I apply clobetasol- there as well- so I have already purchased the medication- but I have not used it due to concerns about side effects- what will happen if I do not apply the clobetasol on the lichen on the lichen my leg?

Detailed Answer:
Yes you can use clobetasol propionate cream on the vitiligo patch for 2-3 weeks and then taper it off.
On long term use it can cause atrophy of the skin.
Otherwise it won't have systemic side effects.
If you don't want to apply clobetasol propionate cream you can use tacrolimus 0.1 percent ointment.
You should definitely use clobetasol propionate on the lichen of legs as steroids are the treatment of choice for lichen.
Topical steroids don't have much side effects if used properly under the supervision of the doctor.
If you won't use it then itching and lichen will persist.
