Suggest Treatment For Vomiting And Fainting Spells When Treating Bronchitis
Question: Hi my grandmother is 86 yrs old and has had a couple of episodes recently. She is just getting over bronchitis and a couple of days ago she vomited and then passed out so we brought her to the hospital and they admitted her for 2 days but didn't find out anything. They released her today and right as we got home and walked in the door she passed out again so the ambulance came and they checked her vitals and blood sugar and everything was fine. Any ideas as to what might be going on? Also she is diabetic.

1. Possible syncope secondary to carotid artery stenosis (slight blockage). This can cause hypotensive episodes.
2. Hypotension due to dehydration.
Postural hypotension needs to be thought of.
As her blood sugar is normal, and vitals good, after recovery, further evaluation depends on blood pressure being checked along with changes in posture.
Does she feel dizzy, during sitting to standing or sleeping to standing (when done immediately).
Due to hospitalization, she could be dehydrated and also additionally, slight blockage in carotid artery could be responsible. In Elderly, blood pressure takes a bit longer to adjust during change of posture. Possibly, when she got home, as she got up from sitting to standing or lying down to standing, her posture change could have triggered a syncopal attack or passing out.
She will require:
1. Good hydration
2. Slow changes in posture.
3. If on hypertensive medication, dosage changes maybe necessary.
4. Venous stockings to improve circulation could be helpful.
