Based on your query, my opinion is as follows:
1. Possible syncope secondary to carotid artery stenosis (slight blockage). This can cause hypotensive episodes.
2. Hypotension due to dehydration.
Postural hypotension needs to be thought of.
As her blood sugar is normal, and vitals good, after recovery, further evaluation depends on blood pressure being checked along with changes in posture.
Does she feel dizzy, during sitting to standing or sleeping to standing (when done immediately).
Due to hospitalization, she could be dehydrated and also additionally, slight blockage in carotid artery could be responsible. In Elderly, blood pressure takes a bit longer to adjust during change of posture. Possibly, when she got home, as she got up from sitting to standing or lying down to standing, her posture change could have triggered a syncopal attack or passing out.
She will require:
1. Good hydration
2. Slow changes in posture.
3. If on hypertensive medication, dosage changes maybe necessary.
4. Venous stockings to improve circulation could be helpful.
