Brief Answer:
Peritonitis should be investigated and treated immediately for.

Detailed Answer:
Hi.
Thanks for your query, albeit short and specific.
Noted the history and understood your concerns.
Any movement, laughing, coughing increasing the pain in abdomen is akin to rebound tenderness and unless proved otherwise is indicative of peritonitis.
You also have diarrhea and pain getting sharp on urination and bowel movement.
Hence I would advise you the following:
GO to ER or nearby hospital for clinical evaluation, tests of blood, stool, urine and ultrasonography along with X-ray of the abdomen in standing position to see for gas under diaphragm, fluid in abdomen .
CT scan if required.
This will give you the correct diagnosis and a plan for management.
Please do not take anything orally and insist for admission so that a proper diagnosis and management can be done asap.

I hope this answers your query.
