Question: I have ovarian chocolate cyst of size 6cm. can I get rid of this without operating it.

Hello and welcome,

Chocolate cysts occur due to endometriosis and are collection of the menstrual blood inside the abdomen forming a cyst.

If they are small and detected early then treatment of endometriosis may help in decreasing thier size or completely eliminating them [ rare].

Your cyst is already 6cm which is not a small size by our standards. So it is less likely to regress by drugs. The medical treatment options are drugs like danazol and GnRh agonists or oral contraceptive pills. The success rates with all the above are variable depending upon the extent of the disease. Mostly vary from 50-30%. Also all of these drugs do have some or the other side effects like male pattern hair growth or loss of bone thickness if used for long term.

For women nearing menopause and completed families we recommend a hysterectomy as it not only removes the cyst but also prevents recurrence.

So consider all these options and also discuss them with your doctor and then decide the best plan. If you are reluctant for surgery then you may try the drugs for 6 months and assess the response.
