Brief Answer:
You need to worry.

Detailed Answer:
Welcome To HCM. I am Dr Anshul Varshney. I understand your concern.
Such a high BP at your age with prexisting Diabetes and Hypertension is a reason to worry.
You would require to get it under control.
I would first of all like to know your Kidney function tests.
Moreover with that Mention your previous BP readings and since when you are getting these readings.
Mention the kidney function report with it's date.
Revert with the asked details for a better help.
