I believe that fungus or toxins that fungus produces is part of the problem of my blood sugar rise as when I use antifungal/antibacterial tablets such as Caprylic Acid with Grapefruit Seed extract, my blood sugar does not rise overnight or at other times when I do not eat. If I do not use them, then my blood sugar will rise even if I do not eat as my liver seems to be putting out too much sugar as though it is feeding something like a fungus.
3. I understand that rice and corn can also contain harmful fungal toxins--what other foods should I stay away from. I know I should not eat flour, high carbs, sugar.
4. My blood sugar went up 50 points from eating plain white fish (around 3 oz) sautéed in a little coconut oil (very little) with unsweetened coconut. This would not have happened with 3 oz of tuna fish from a can made with onion and celery.

It looks like I need a customized diet with some options, so that I can remain on it for life, instead of the yoyo other diets that do not help me for one reason or another in the long run. The food choices do not seem a lot. I love rice and beans with vegetables and I could just eat that forever, but I cannot because I am a diabetic.

First you need to get your lipid profile, thyroid profile, microalbuminuria and HbA1c checked. Also keep record of your fasting and post-prondial glucose record every day for a month in your diary with a well calibrated glucometer. Your medications are sufficient for time being if you are committed for lifestyle changes.
First stay away from all the fast / processed and fried food stuff. Divide your meal in to 4-5 parts per day whenever you feel hungry. With 2/3 snacks in between. That helps to keep the sugar levels constant throughout the day. Add more of fiber rich food stuffs like green leafy vegetables, raw fruits (not juices), take very less of carbohydrates (rice) avoid sugar completely. Take Millets in boiled form or the breads prepared out of it like ragi, bajra, sorghum, wheat etc., are very good source of micronutrients required for the action of insulin. Take more of protein rich food, like egg white, fish is good source of omega 3 & 6 fatty acids. Moreover, beetroot juice (inorganic nitrate), olive leaves, garlic, turmeric, tamarind also have proven benefits. Say no to oils and cholesterol rich food stuffs. Based on this, with the help of a dietician, you prepare your diet chart and strictly follow it.
Physical activity is very important, play games, walk at least half an hour to one hour a day. This not only helps you to burn calories, but also to relive your stress. Music is another way to relax too. Try and practice yoga, meditation and pranayama. Some aasanas are very easy to practice and also beneficial. Quit smoking (if) and reduce your alcohol intake.
