Brief Answer:
Yes, you need a prompt medical help.

Detailed Answer:
Hello and thank you for asking!
From your description i can understand that the situation is severe, there many pathologies to be treated. The blood pressure 220 is to high and the blood glucose needs intensive treatment to get lowered.
The difficulty in breathing is another serious problem that needs to be evaluated further.
The high blood pressure can cause it, but anyway cardiac or pulmonary pathologies should be excluded.
So, i do recommend to go to urgent care to take appropriate treatment for each problem.
Wish you good health.
Kind regards,
Dr.Mirjeta
