Your problem may be due to urinary tract infection(UTI) as you have pain,increased frequency and significant post void residual urine.
It is important to know other details of ultrasound like kidney status.
With these findings,I suggest urine RE/ME (routine examination/ microscopic examination),Urine culture and sensitivity to my patients.
I also suggest nitrite test and leukocyte esterase test for confirmation of UTI.
