Suggest Ways To Regulate Blood Sugar Levels
Question: Hello doc
I am diabetic since 10 years my sugar is not under control from 3months the medicine dosage is
glyciphase 850mg 3 times a day
diapride 3mg in the afternoon
when I asked my doctor regarding splitting the diapride in the night he said no diapride is not suppose to be taken in the night
my problem is only with the fasting sugars as my random sugars are in control
I would like to know can I increase the dosage to 4mg and take 2mg diapride in the afternoon and 2mg in the night
and can diapride be taken in the night

You are currently on maximum dose of Metformin. Diapride ( glimeperide) is usually a single time ( morning time) pill. However, occasionally I split the dose and gie a small dose in the morning. Your doctor is concered about the possibility of low sugars at night if Diapride is given at night. May i kow your recent FBS, PPBS and HbA1c values? I can advice the dose accordingly.

Fasting sugars are in and around 180-150 from 1 week today the fasting was 150 and yesterday the ppbs was 190
Hba1c value is 8.25
And I have irregular periods too.

If cost is not a concern, i can advice you following combination.
Diamicron-MR 30 mg once in the morning before breakfast.( instead of Diapride)
JanuMet 50/1000 one tablet twice a day. ( instead of Glyciphage).
However, if cost is a concern, you may take Diapride 3 mg morning and 1 mg at night ( 10 minutes before food) and continue Glyciphage ( metformin) 850 mg three times a day.
I hope you had a kidney function test and cholesterol profile done recently and they are normal. What about blood pressure ?
Your Hba1c is certainly high and need to bring it down to less than 7 and close to 6.6%.

It is a wrong concept that diabetes medicines can harm the kidneys. It is the uncontrolled diabetes which harm kidneys, not the medicines. However, we always advice people with diabetes to avoid pain killers which can harm kidneys.
If cost is not a concern, you can take the medicines as advised. You can stop Diapride and Glyciphage .
Diamicron is only in the morning and JanuMet 50/1000 one tablet in the morning and one at night.
In fact, it is not nice to prescribe medicines like this with out seeing you. Ideally, you should visit an endocrinologist. May I know from which place you are so that I can advice you about the endocrinologist available near by.
