Question: Swirling pain in chest that comes and goes throughout the day in left side of chest. Soreness in throat difficulty swallowing. Extremely uncomfortable. Went to Dr and they diagnosed with acid reflux and referred me to take Nexium. After 3 days it seems to be getting worse. Does this seem like the correct diagnosis?

Brief Answer:
Doses of Nexium is important. The diagnosis seems to be correct prima fascia.

Detailed Answer:
Hello and thanks for choosing 'Ask A Doctor' service,
I've read your details and I appreciate your concern.

Your history is suggestive of gastritis. It's a condition in which there is swelling in the soft inner layer of stomach and esophagus(due to reflux) causing inflammation and pain.

Cardiac cause at an age of 35 is extremely rare unless you have a very strong family history of Coronary artery disease or you are a chain smoker.

I would suggest you go through the following tests-

Endoscopy to rule out inflammation in the stomach. Ulcers could also be there which may cause pain.

In endoscopy, they are going to put a camera inside your esophagus via mouth to look for swelling.

If normal go for 2d echo and treadmill test. Combined together they could rule out cardiac involvement in 80% cases.

Rx
You have not mentioned the dose of Nexium you are on,
I would suggest you take it 40 to 80mg twice daily for the initial 1 week.

Please remember to take the medicine empty stomach. You should not eat anything after taking it for 30 minutes.

Along with it take an over the counter composition of an antacid containing Magnesium hydroxide and aluminum hydroxide 2 tsp twice daily for a few days.

In case you can manage to get Acotiamide tablets 100 mg and use them thrice daily. Not available in the USA but you can get it from Mexico or Europe easily.

Even after this protocol you don't feel well go for cardiac tests as mentioned above.

Hope I was easy to understand and helpful.
Follow-ups are welcome.
Thanks!

Brief Answer:
follow up.

Detailed Answer:
Hello and thanks for follow up,
No its not really a rare case.
Anxiety can cause any problem in our system. In your case anxiety stimulates your nerves therefore the rate of contraction and relaxation as well as regugitation starts causing severe heart burn pain and acid peptic problems.
If left untreated for a long time it may even cause ulcers to develop along the gut.
You have not mentioned earlier that you have issues with anxiety.
Start taking Natural supplement Lactium twice daily to curb anxiety.
In case you are not relieved then you have to go for prescription medicine like clonazepam,Etizolam etc for few weeks along with antacids.

Also engage yourself in yoga breathing exercise early morning walk. They all would help you to deal with anxiety.
Hope i was helpful,
Thank you.
