The Haematologist Gave A Mixed Diagnosis. He Suggested The Fact
![default](/r/images/default.png)
Question: The haematologist gave a mixed diagnosis. He suggested the fact I had low iron previously but no elevated platelets indicated he feels it’s essential thrombocytosis. For the past three years the range is between 480,000-520,000 with low iron and ferritin levels.
Do you believe it’s essential thrombocytosis given such low platelet levels or is it related to low iron levels?

Brief Answer:
Iron deficiency needs to be corrected. Inv for cause of thrombocytosis.
Detailed Answer:
Hello,
Thanks for posting your query on Ask a Doctor forum.
Iron deficiency state is usually associated with elevated platelets.
Low iron and low ferritin suggest low iron as well as low iron stores.
You need to take iron supplements till the iron level comes back to normal.
The platelets are on higher side for last three years and need investigation.
I suggest you to consult your hematologist and get certain molecular tests namely : JAK 2 and CALR mutations.
Bone marrow aspiration and biopsy may also be done to assess the status of megakaryocytes and megakaryoblasts.
Thanks and take care

We have completed the jak 2 and CALR mutation and this came back negative.
Yet, he still suggests I have essential thrombocytosis. Do you agree with this diagnosis?
I am going for a bone marrow biopsy on Monday.

Brief Answer:
MPL mutation can be added to bone marrow examination
Detailed Answer:
Hello and welcome again,
I suggest you to get MPL mutation (if available) also since JAK 2 and CALR mutations are negative.
You can proceed for bone marrow aspiration and biopsy.
Final diagnosis can be made after all investigations are completed.
Thanks and take care
