Question: The property management firm that I am applying to live at has asked me to provide sufficient documentation from a licensed physician in support of my ADA accommodation request. I have had epilepsy since I was 8 years old. Specifically, nocturnal seizures (tonic-clonic). My service dog is essential since he is in charge of waking me before a seizure begins, allowing me to lay flat on the floor in a safe location. He also alerts my family so they can monitor me during the seizure. Since I no longer have a PCP (lost job & insurance), is your physicians able to provide documentation supporting the use and need of my service dog now that I have to move into a rental home after loosing my home? The property management firm has asked for a physician’s note, on their letterhead, stating my need for a service dog as essential for my condition. I have already provided the other required documents for the animal, the final document needed is from a licensed physician stating the need. I have no problem releasing my information regarding epilepsy as the root cause of the service dog use in the letter. Thanks

Can you suggest a Neurophysician to consult with on your team?

I am simply looking for a note for the service dog I own that helps keep me safe when I sleep. I have really injured myself in the past going into a seizure and fallen out of bed, hitting my head. I'm asking for a note stating the need for my service dog. If I had money or insurance to seek a local Neurophysician, I already would have done that. Thanks
