Brief Answer:
There is one area where there is a small suspicion of hiatus hernia.
Detailed Answer:
Hi,
Thanks for sending the images.
There are sagittal section images sent in mail.
There is one area where there is a small suspicion of hiatus hernia.
Image link is below and I have marked in a blue circle.
https://ibb.co/tbH9fXT
I wish to see the same area in coronal images if it is available.
Regards,

Hi Dr XXXXXXX Cail
Can you please tell me what the ball at the pylorus is?On EGD it showed congestion and inflammation as well as mucosa at d2 duodenum.
I also wondered if the ileum (or the part I pointed the arrow at on his right side) is herniating through the mesentery. It looks odd to me.
The last question I have is why his colon has a 3 ribbon gas look by the spleen.
Another radiologist said his colon looks like it has a stricture in the descending colon and stated that his bowels look grossly abnormal.
Could that be IBD or, possibly caused by hypertension from hiatal hernia?
There is certainly hiatal hernia. On the scan done with contrast you can follow one of the ribbon looking gas patterns to the diaphragm and a little light appears above it.The same holds true for a small part of the stomach and left kidney. They are little but he has holes in his diaphragm ( or 1 hole it is all passing through.A small part of the liver is seen going into diaphragm as well. You can see these things in this no contrast scan as well if you look closely.
Thank you for all of your help, just want my son to be able to eat and have normal bowel movements. He has always been reaally energetic but the last 2.5-3 years he is tired all of the time and feels worse than normal with more puking and intermittent blood in stool.
They said he has lymphoid hyperplasia of termial ileum, if that helps.
Thank You

Brief Answer:
There is suspicious mucosal irregularity in the pylorus and duodenum area
Detailed Answer:
Hi,
Thanks for writing in to us.
There is some mucosal irregularity and edema like area in the part where you mention about the ball at the pylorus and duodenum area. It is matching with endoscopy details and might suggest inflammation.
There is no significant herniation of the ileum.
The colon is a flexible part of the bowel and gets distended with fecal matter and gas as and when needed and is collapsed at other times. The ribbon looks is not significant.
I do not find any stricture in the given images. However proper distension of colon is needed to confirm that there is no stricture in the descending colon area.
Regarding holes in the diaphragm, I find only one foramen through which the esophagus enters the abdomen and there is a small hiatus hernia.
Lymphoid hyperplasia is a benign condition involving lymphoid nodules in the lower part of the small intestine and this needs to be confirmed by an endoscopy.

Hi Dr Chail
Here is an ultra sound and different mrcp. Please check for compression syndromes such as SMA.It looks like there is a blister on bile duct in different scans he has had. Please also check for hernias.
Thank You

Brief Answer:
There is a possibility of SMA syndrome
Detailed Answer:
Hi,
Thanks for writing in to us and hope XXXXXXX is doing well.
There are 79 images of ultrasound scan and in image 13/79 three is visualisation of an acute angle of the SMA and is a possible cause for SMA syndrome.
The MRCP has 72 and 4 images in 2 series. In the 4 images there is a mild bulge in the proximal common bile duct in image 2/4 and is likely not significant.

Hello Dr XXXXXXX Chail
New Report attached
This is secretin scan done with water contrast. If you look at Dixon post images (especially in sagittal frames; I think it shows sma syndrome well as well as the hiatal hernia.
If you look at the ducts in 3d you can see the duct of wirsung going all the way around d2 junction duodenum then, it joins ampula of vader. There are also 2 tiny non fusing ducts. Please comment on which frames you see the ducts in.Another radiologist found them on this scan I am attaching where they are from a ct scan to give you an idea where to look.
Please advise and Thank You for your time.

Brief Answer:
The non fusion of ducts is likely artefactual
Detailed Answer:
Hi,
Thanks for writing in to us.
I have seen the images in detail and discussed with another radiologist and both of us find that there is no obvious non fused ducts and the appearances are of artefactual nature and not real non fusion of ducts. The duct of Wirsung is around the D2 segment however there is no duct dilatation.

Thank you for the opinions Sir.I am emailing an image from ct scan that the other radiologist says are 2 non fusing ducts. There is something that looks like 2 tiny lights connected to 2 strings crossing. This is what they said were non fusing ducts. They said they could not find it on secretin scan at first but, notified me that they found it later.
Do you have a guess of what else it may be? Thank You

Likely thin ducts in the area
Detailed Answer:
Hi,

Sorry for the delay in replying.
The non fusion appearance can be due to thin ducts and which are ordinarily not visualised on the scans. I feel if is more likely to be thin ducts than non fused ducts.
