This Is Very Embarrassing For Me To Ask, But I

default

Question: This is very embarrassing for me to ask, but I need to know about my condition and if anyone else on the planet has this, what I can do about it, etc. I've never told anyone about this -- not even my doctor. I'm too embarrassed to say it in person to anyone. Here we go: More often than not, cleaning up after a bowel movement is a project. If using TP, my anus will get bloody long before it is anywhere near clean. I must use wet wipes; however, even with these, it can take a ridiculous amount. I'm lucky if I'm on and off in 10 minutes. I wipe and wipe and wipe and there's more and more and more to clean. It's as if the feces didn't fully vacate my colon, so even after a large BM, there's a small but messy chunk in there. Not using wet wipes (and a lot of them) results in a messy, itchy, and eventually very sore rear end. Before you rattle off the normal checklist, I must also say that I have a very healthy diet (no soda or alcohol, lots of water, black coffee, fruits, and 100% whole grains and oats). Nothing makes much of a difference. I saw a GI specialist (a very good one too) a few years ago for an unrelated problem. I needed a colonoscopy. I did not tell my dr. about my other issue; I just hoped he would mention finding something unusual while he was doing the procedure. He said everything was healthy, and the only thing I have is some small hemorrhoids. I'm very healthy by all other measures and I'm under 30 -- my first memory of this condition was when I was 11 -- I've had it ever since. So everything appears normal to a GI dr. observer, my diet is overall very good, and I have no other known underlying conditions. However, I can't travel with any sort of comfort ANYWHERE without a sizable supply of wet wipes. It sounds like a minor inconvenience, but it's actually quite the burden to live with, especially when you get yelled at at work for "taking so long," or when you hold it in because you're away from home without a wet wipe supply. First, I really want to know if you have ever heard of what I'm describing happening for any other patients. Second, is there anything else I can do other than accommodate it the way I currently am? I would apologize you having to read this gross description, but I'm sure as a dr., you've read worse.

Hello and thanks for choosing " Ask a Doctor" service for your query,
Have gone through your query and I appreciate your concerns,
Very likely he problem you are having is called fecal incontinence.Fecal incontinence is a usually a result of impairment of anal sphincter muscles which surrounds the anus. In some people's these muscles get weekend up resulting in continuous drooling of stools from anal canal the blood that you are taking about is very likely from the. hemorrhoid you have .
A MRI anal sphincter should be done to check the patency of anal muscles.
Hemorrhoid should be treated conservatively initially however if not cured surgical excision could be done.

Hope I was helpful,
Thanks.

I don't see how it could possibly be that. I have full and normal control of my bowels. I never have "accidents." I can "hold it" as long as anyone. The problem happens after I choose to go. Then the cleanup is a project.

Detailed Answer:
Hello again my dear,
The first part of your problem that is blood which have to wipe and wipe for many times is definitely because of hemorrhoids. So it's a time when they should be excised off at least this would cure half of your problems.
Second part of problem is the seepage of fecal material post evacuation.Though this is usually rare as because as per your explaination your bowel moment is under your control but once you are done it keeps on seeping. I would suggest you to go for a fecogram in which your pre intra and post bowel events are recorded and this would provide an idea of the problem going on. Secondly get an MRI done to see if sphincter is really working properly or not post defecating.

Needs these tests reports to help you.
Get your blood sugar levels fasting checked up also.
Discuss this with your treating doctor and revert back with what he suggests you .
Thanks.

Hello again,
At first I would suggest you to take care of your hemorrhoids. Its very likely that after motions they prolapse outside hence creating a small space for leakage of mucous and left over stools.
Initially get them operated and see if the problem still persists.
If the problem is still there then we would think about sphincter problem etc.
Hope I was helpful,

Thank you! Is there any treatment for this condition (other than the hemorrhoids), or is it likely that I'll just have to keep managing it by carrying a stash of baby wipes everywhere? I'll also note, when I had my colonoscopy, the doctor said everything was normal (except small hemorrhoids). I'm pretty sure I developed hemorrhoids due to this problem; as a child, I didn't know better so I strained to try to squeeze out what was left.

Brief Answer:
Fully treatable.

Detailed Answer:
Hello again,
Your squeezing has caused the hemorrhoid to bulg up more.
Basically hemorrhoids are dilated veins squeezing them has caused the problem to aggregate.
I would suggest you to get your hemorrhoids checked up again.
There are four stages of hemorrhoids and every stage is managed differently.
This is a fully treatable condition and very likely the problem of leakage is a secondary complication of this.

If you can then please attach a clear photograph of the area for my opinion.
You can use your smart phone but image should be clear you can edit the image to mark the area where you feel the hemorrhoid is(if not prolapsead).

Thanks.

In the event that getting my hemorrhoids surgically removed doesn't solve the problem, is there anything that will solve it, or will I likely just have to cope with it?
