Question: hello doctor,

hello doctor i experienced syptoms like i forgot things i experince like i am in my 28 age but i feel like 40 i am tired when i think or say something i think more effort is required to say what i am saying i have also stomach problem i hate noise and get distrub from noise there is thinking of deprived or as i am a loser and one of my feeling of that thing is just not in my life like i cant get a girl to have sex like these feelings hit me so much and sometimes negative feelings come to much i feel tired when i do little work i feel i am no more a young person like i used to be and when i am with friends i talk with them i feel happy.

Thanks for your question.

I understand that you seem to be going through a difficult time. The symptoms which you have described (i.e. feeling dull and disinterested, tiredness and fatigue, negative feelings, etc.) may be suggestive of depression, but they definitely need further exploration.

I would like to know some more details regarding your symptoms, in order to understand your problem better.

- Since when have you been feeling this way?
- How did the symptoms start... i.e. what is sudden or gradual?
- Have you recently experienced or are currently experiencing any stressful situation or major changes in your personal or social life or at work?
- Are these negative thoughts persistent and generalized or only in response to certain situations or negative events?
- How is your sleep and appetite?
- Have you felt that your future is pointless or your life is not worth living?
- Are you in the habit of taking alcohol or using any other substances?
- What anti-depressant medication were you on and did you notice any improvement with that?

Please do get back to me with these details, so that we can take this further and see how best I can help you.

number 1 from last three years or so
2 dont know exactly also
3 no there were good changes i had a good job and good life apart from death of my father that was in 2005 and it was not that stressfull to me.
4.only to certian situations.
5.sleep is good and appitite is good too.
6.no but future stress or future worry happens.of either job losing something losing job.

8.i exactly dont know about them but they seem to get me tired and i used to sleep much but i took it for 3 months because i felt i am healthy actually the doctor here in pakistan misdiagonised me as i was quite healthy and i had to go to doctor only for anger and my mother was to illatrate to get me to a phsyctarist you know there is a trend here to give medicine to feel better if there is no illness in pakistan.

from last three years i have situation like that i am trying to control some agressive images that are in my mind that started when i read an article on the internet about repressed images so some agressive images were there with me for 1 to 2 years and i was trying to stop them like yelling agressively in images

Hi again and thanks for writing in.

After your detailed description, I feel that though you may superficially seem to have depressive symptoms, the underlying core problems are likely to be a lack self-confidence, negativistic attitude and self-reproachful ideas about yourself, thereby resulting in excessive worry / anxiety.

You have mentioned that your sleep and appetite is fine, that you are happy when you are with your friends and that you get these negative thoughts predominantly in response to certain situations. All these are not in favour of a biological depression or a major depressive disorder.

Rather, the negative feelings, disinterest or lack of motivation you are suffering from seem to be a result of your dysfunctional negative thinking pattern... and you seem to have many negative thoughts, like 'I am a loser', 'I am not good enough', 'I may lose my job', etc.

I think that you would benefit a lot from psychological therapies such as Cognitive Behavioural Therapy (CBT) - this form of therapy is very helpful for such negative and demotivating thoughts and help improve your confidence levels. I would suggest that you consult a psychiatrist and discuss about this option. There are also a lot of resources available online to combat such negative thinking and improve your confidence levels. Online CBT is also available in certain websites and if you live in England, your GP can prescribe you free access. Here are some good online resources for your problems:

Now, the other symptom which you have added is the repeated and intrusive aggressive images, which you are finding as distressing and difficult to control. From your description of such images being repeated and intrusive, it may be that you are having obsessive-compulsive symptoms also. If so, this can be adding on to your anxiety and distress, and you need treatment for that also (which can be with medication or psychotherapy).

I would advise you to consult a psychiatrist (preferably one who does CBT) who would be able to assess you in detail and formulate a systematic management plan for you.
