Question: To be answered by Dr. Prof. XXXXXXX Saha,
Doctor Saha, this is Rosemarie Raia.
I have been prescribed gabapentin for the numbness in my feet and calves and buttocks due to a bulging disc possibly pressing on several nerves.

Is there a natural alternative to gabapentin. The gabapentin makes me very lightheaded and four days ago after having taking it the night before, the next morning I was ok upon awakening but when I laid down for 10 minutes and tried to get up the room was spinning.

The night before I did not take my second candesartan 16 mg. because my bp was 107/77/

The doctor stopped it for four days . If there were an alternative I would be unafraid to take it. I am not walking so steady now because of the numbness in my feet and the dizziness added to that is very difficult. Thank you.
I resumed the gabapentin for 8 days. I had morning headaches which sometimes left me with a dullness in my head for the day and sometimes a numbnes in my head and face.
My doctor stopped the gabapentin for 10 days ending on May 13. He wants to see if the headaches stop while I am not taking it. They have not gone completely away but are much lighter and today not at all.
The numbness in my feet seems to be worse.
My doctor does not think it is the gabapentin and wants to put me back on it after May 13th. I am not certain what to do..
Can you explain "centralization" in physical therapy for the back?

My doctor does not think the headaches are coming from the gabapentin.

I am having worse numbness in my feet and it is difficult to walk.

I am still going to physical therapy and feel better when I am there. I need to understand centralization in physical therapy and if and when I should restart the gabapentin or is there something better in my case.

Brief Answer:
Discuss with your treating physician before reinitiating gabapentin

Detailed Answer:
Thanks for choosing to ask me once again.

I am not aware of any natural alternative to gabapentin.
Centralization is a term used in a form of physical therapy known as the XXXXXXX Method of Mechanical Diagnosis and Therapy (MDT). Centralization describes a phenomenon whereby pain in an arm, leg, or buttock suddenly shifts to a spot closer to the spine if the spine is either moved or manipulated. It typically suggests a less serious problem of the spine.

I think that your doctor is doing the right thing by trying out alternatives. The only problem is that the pain and numbness of the lower limbs is likely to worsen. I would insist you to discuss with your treating physician before reinitiating gabapentin.

Dr. Saha,
Why would the pain and numbness of the lower limbs worsen? What would make it worsen? I will talk to the doctor before reiniating the gabapentin. What do you think I should do?

By alternatives, do you mean the physical therapy, or the gabapentin? I do not like the way it made me feel with the dizziness and headaches and the warnings. If I thought it would help the numbness I would try it again but I read it is for pain, and nerve damage. I do not know if I have nerve damage.

Brief Answer:
I was talking about gabapentin. Physical therapy needs to be contd.

Detailed Answer:
Gabapentin is expected to relieve the pain and numbness. Thus, stopping gabapentin can make the pain and numbness come back.

There is nerve compression involved as the cause of neuropathic pain. Not sure about the nerve damage.
