default
Posted on Mon, 14 Oct 2019
Question: Today is day four of eating a very low caloric diet. The first three days I had less 500 calories per day and so far today I have had 180 calories. Dinner will be 120 calories for a total of 300 calories. However, even though I have had almost nothing to eat today, I am still bloated and have only lost two pounds. What could be causing my distended abdominal area?

Brief Answer:
Unfortunately, such low calories have no role in curing your problem.

Detailed Answer:
Thanks for your query on 'Ask A Doctor' service.
I have gone through your query.

I am sorry but I cannot understand the relationship between such a low-calorie diet and your bloating problem. Kindly mention in follow up what your doctor said while telling you to take 500 calorie diet.

For me, your problem seems to be due to your gut. If somehow the gut movement gets hampered, food along with the digestive juices would stagnate and the bowel habits would not be proper. Degradation of undigested food in the intestines by the intestinal bacteria would lead to excess gas formation.

Secondly, the 500 calorie diet is too harmful.
There is a thing called BMR.
It's the minimum amount of calorie required by the body for your vitals organs to work as the heart to beat, lungs to assist breathing.
Calculate your BMR and take at least that many calories (you can simply search for BMR calculator and do it on your own).

In order to relieve yourself, please to do the following as well:

* Avoid spicy food. Take small but frequent meals. Avoid long gaps between meals.
* Avoid alcohol intake (if it a part of your habit).
* Take omeprazole once daily before breakfast. XXXXXXX Econorm a probiotic three times a day.
*Tablets of activated charcoal thrice daily to absorb gases.
* Take plenty of green leafy vegetables as well as other sources of dietary fibers. Take fiber supplements like ispaghula/psyllium husk or senna. I would insist you to increase your dietary fiber intake.

* Regular morning walk and exercise to keep the bowel movements proper.
* Take very light easily digestible home-cooked food...
* Take curd or yogurt on daily basis. It contains beneficial bacteria and should help.

* Stress reduction has a very important role to play. Yoga and meditation should help.

Hopefully, that should help.

Let me know if I could help further.

Regards

Brief Answer:
Follow up answer.

Detailed Answer:
Hello again my dear patient,
I can very well understand your problem but moving a person to 500 calories a day will cause you more harm then gain.
Your bloating is relate to SIBO.
There are a number of colonies of bd bacteria yeast and fungi inside your intestines which causes severe inflammation. This inflammation leads to improper digestion of food. This causes fermentation of food leading to release of toxic gases like CO2 hydrogen sulphide etc resulting in bloating.
Nexium is an antacid and not related too much to bloating.
XXXXXXX Econorm a probiotic three times a day.
Take very light easily digestible home-cooked food.
Take a laxative daily.
Simethicone two tablets thrice it allows gas bubbles in the stomach and intestines to come together more easily, which allows for easier passage of gas. Simethicone is used to relieve painful pressure caused by excess gas in the stomach and intestines.

follow up in 5 days you would be far better,
500 calories are too less.

This is the minimum amount of calorie you need to take-

Actually, my doctor is indirectly involved with my Caloric Restriction diet. I went, once again, to my gastro last week for the bloating. We have tried Nexium, we have tried Xiafaxan, we have tried the FODMAP diet. Nothing was all that effective. Last week he said all he could think of now before we go to an endoscopy is losing 5 to 10 lbs and cut down on the stress. My prior weight loss attempts have been unsuccessful, so I decided to go on a CR diet. I do experience chronic constipation, but I just had a large BM and feel much better. I also tested positive for SIBO two years ago, and have a hiatal hernia so that might be factors. I have heard and read quite a bit about CR and fasting and the health benefits, hence, my CR diet.

One last question. I am flying tomorrow and I always seem to get very gassy on flights. Are there any foods I should avoid prior to the flight? I do take charcoal pills and GasX prior to and on the flight.
