Question: We are based at a remote place at Tikiri a place 60 km away from Rayagada District of Odisha. My mother who is of at around 75 years of age and she has been suffering from prolong back pain and on the same aspect we visited the Doctor at XXXXXXX

As per the Opinion of the Doctors they suggested for spinal Surgery. So request you to please guide on the said problem .

Will be very much thankful for your advice. For Your reference the MRI report is attached herewith along with the Doctor's prescription. Looking at her pain request you for the best option available. Her Medical Details are enclosed for your reference.

If Surgery required what Surgery needs to be done and which Levels to be addressed.

I have reviewed patient's symptoms and MRI spine reports.

I agree that surgery would be beneficial in relieving pain.

Surgery required would be spinal lumbar canal decompression (discectomy+ laminectomy) with fixation. It would be an open surgery.
As per the MRI report, the significant involvement is seen at L1-L2, L2-L3 and L3-L4 levels, so, these levels need to be addressed.

Same medicines can be continued.

Regarding the risks, she would need assessment by physician, cardiologist and anesthetist to assess her fitness for surgery. If she is found fit, the risks are minimal, which would include a mild risk of infection at the site of surgery, numbness or weakness of foot/leg due to injury during surgery and bleeding. The risk of all these put together is usually less than 5% of all patients.

As of now my mother is getting vigorous pain what medicine needs to be taken to reduce her pain
She is unable to walk property

Further we are planning to travel for her surgery next week what precautions we need to take. till her surgery. We are carrying out her physiotherapy. And medication as prescribed.

Current medicines are dine. You can give additional ULTRACET tablet as and when needed for pain (maximum two per day).

Journey needs to be smooth, without major bumps. You can ask her to wear lumbar corset (jacket) for providing stability to the back during travel.
