What Are Remedies For Second-generation Antipsychotic Medications?

Question: I am coming off the opioid Oxycodone with the help of CBD oil like I used to come off Diazepam in February 2018. The CBD was interacting with my risperidone and I had to stop it abruptly. It has been 3 days without a 4mg risperidone, and I want to know how much longer untill I should expect the delusions, ect., for I have been on it since April 1990?

Risperidone belongs to a second-generation antipsychotic medications. It adjusts neurotransmitters dopamine and serotonie in the brain to improve mood, thinking and behavior.
CBD oil decreseases the activity of liver enzyme (cytochrome P-450) which is responsible for risperidone clearance in the body. Therefore, CBD oil increases the dose of risperidone (by decreasing drug clearance) and may cause side effects. Please, talk to your doctor and discuss decreasing the dose of risperidone while you are taking CBD oil.
When you stop taking Risperidone it is cleared from your body after around 2 days (max half life 20 hours) and the balance of neurotransmitters in your brain starts to change again. You may get your old symptoms (e.g. delusions) but it is really difficult to tell the time when will that happen. Even you may get withdrawal symptoms but risperidone’s withdrawal symptoms are rare and usually mild.
Briefly, it is difficult to say when symptoms recur after stopping risperidone. I advise you not to stop it. Consult with your doctor to adjust the dose of risperidone (decrease) while you are taking CBD oil.

I have Bipolar 1 and I have stayed faithful to a medication regimen for 28 years. I also started the opioid Hydrocodone 7.5 in 1990. I quit the narcotic Diazepam in February 2018, and I am determined to come off the opioid Oxycodone now that I have real pain relief with CBD oil 4 drops every 12 hours and Lyrica, 3 150m. capsules per day. My risperidone was reacting with the CBD oil and causing restlessness and very bad temper. At the same time, I was receiving anxiety relief. Is the Zoloft, 2-100mg. mornings, Topiramate 200mg twice a day, Nortriptyline 50mg. at midnight taking the place of the Risperidone 4mg. that I have not taken now for 4 days?

Unfortunately, bipolar disorder is a lifelong illness that requires maintenance pharmacotherapy to reduce the risk of recurrent mood episodes. Depending on severity of the disease course most patients require maintenance therapy for many years or a lifelong maintenance.
Cannabidiol (CBD) oil from cannabis plant is considered safe and non-addictive. It is widely used to treat pain, anxiety and inflammatory conditions. Concurrent use of CBD oil and risperidone caused you side effects (restlessness, agitation or anxiety) because of increased concentration of risperidone in the body. Decreased dose of risperidone may help to avoid those side effects.
Recent researches show CBD oil may provide some antipsychotic benefits to bipolar disorder as well. But still there is no enough evidence of CBD oil benefits on the treatment of bipolar disorder.

Topiramate has similar effects of risperidone but Zoloft and Nortriptyline are antidepressants and have no antipsychotic effects of Risperidone on bipolar disorder. The combination of Risperidone and Topiramate is used for difficult to treat bipolar disorder. But Topiramate alone is not approved to treat bipolar disorder.

Therefore, I highly encouraged you to remain on risperidone as discontinuing stable treatment may leads to a new mood episodes within a year.

Thank you for your thoughtful answers. If it were possible, I would get 1mg Risperidone from my physicians and lessen my dosage and proceed in a civilized manner. They do not give me that alternative. I am counting on the 28 years since I was on the Risperdal at 5mg and now, during which time I have had several severe bouts of mania and suicidal ideations in mixed to in the last three years a steadying on a course.
Again, would it help if I took the 4mg. every other day or would it as I thought simply put me on a roller-coaster with Risperidone and CBD? Thank You

I understand it must not be easy for you to take risperidone for 28 years.
I am sorry that I cannot make changes the dose of your medicine. It is your doctor to decide regarding the change the dose of risperidone who is familiar with the disease course, response to the medicine and overall your health.
Taking risperidone every other day is not recommended. It causes drug level fluctuations in the body, and may cause withdrawal symptoms. Risperidone should be tapered gradually taking lower dose of the medicine every day. As you are taking CBD oil it would be reasonable to lower the dose of risperidone as CBD oil causes increase of risperidone concentration in the body when taken concurrently.

Again, I highly recommend you to talk with your doctor about changes in the medication dose.
