Question: What are the steps in your old age, when you can no longer care for yourself? Is it first assisted living then a nursing home, and when are those needed? Is there anything after the nursing home, and how do you tell the difference between the two? In the categories it is not clear, do you have to call them? THank you very much...

I am Dr. Prakash HM, and I would like to answer your questions.
Steps in older age, when the elderly person cannot take care of self includes long term care services like attendant care, home maker services, non-urgent transportation.
If she requires further support
1. Assisted living is usually done when the patient is ambulatory and continent and require assistance to manage their daily activities, medication administration, transport to and from hospital.
2. Nursing home care is usually done when the patient has developed certain complications and require further specialist opinion and standard treatment. Patient needs to be in hospital either in ICU or ward facilities and needs time to recuperate. As patient improves, he/she will be shifted back to assisted living.
3. End of life care is when the elderly patient moves from aggressive intervention to palliative care. During this time, in the nursing home, symptom management, family support, bereavement counseling, and management of physical, psychological and spiritual needs of the patient is done. It is usually done in nursing homes only.
