What Are The PSV Levels In A Patient With No Arterial Plaque?

Question: Recently had a physical which included a PSV done by sonogram. The physician's analysis was "mild risk"The numbers were: left PSV 11-125, same for right. I assume the two numbers are diastolic and systolic. In a person with a complete absence of arterial plaque, what would these numbers be, assuming optimalBP?

Regarding your concern, I would explain that such Doppler velocities of carotid arteries (PSA 125 cm/sec and EDV 11 cm/sec) are considered within normal ranges for an apparently healthy individual.

When no apparent atherosclerotic plaques are identified there is no need to worry about the above Doppler velocities, as they may very depending on blood pressure, blood volume status, heart rate and measurement technical features.

Other measurements such as ICA/CCA (common carotid artery) velocity ratio is evaluated when internal carotid artery (ICA) stenoses is detected.

As far as no carotid plaques are observed, the best strategy to follow would be controlling potential risk factors such as uncontrolled hypertension, dyslipidemia, any glucose metabolism disorders (diabetes, or impaired glucose tolerance), close smoking contact, overweight and physical inactivity.

Coming to this point, a healthy life-style, diet profile and daily physical activity are strongly encouraged.

In addition several lab tests, including:

- complete blood count,
- PCR,
- liver and renal function tests,
- thyroid hormones level,
- fasting blood glucose level,
- glycosylated hemoglobin level (HbA1C),
- blood lipid profile

should be checked for quantifying risk of accelerated atherosclerosis.
