Posted on Wed, 7 Jan 2015
Question: Hi. On December 05, 2014, i had sex with a prostitute from Pakistan in Dubai. The act was for less than two minutes (from penetration to ejaculation) when i pulled i saw the codom was broken and half of my penis was expose. The girl ran to the bathroom and washed herself. She came out and was casual about it. I asked her and she said she is find and looked at me and said you are fine. Also, said she gets herself teststed when she comes to Dubai and also when she goes back. She seemed like a average person who has a life (showed me her facebook and modeling photos in Pakistan). I went to the loo and peed and wiped my penis with hand towel to see if there is any blook or anything. There was nothing. I got back home the next day and had antibiotic for any possible STDs. Its the 7th day today and I dont have any burning sensation when i pee. However, my anxiety is killing me. my mind is telling me I am going to get something (HIV). I called her pinp and he said she is clean and you shouldnt worry. He said before she came to Dubai she was tested. He said i cant take guarantee for the 15-20 days she has been in Dubai, but before her tests were done. My heart is racing and I cant take this any longer. I have a family and small kids and I was so stupid when i did this. I will take the HIV RNA PCR Test tomorrow. Can some one please write and tell me what are my chances of being healthy. I am praying. An early reply would be apprecaited.

Brief Answer:
Minimal or almost nil risk of HIV transmission

Detailed Answer:
Hi
Thank you for query. I am Dr Bharatesh D Basti answering your query.

All intercourses are not risky. The chances of HIV transmission by vaginal intercourse is 1/200 to 1/2000 even if the partner is HIV positive.
Here the possibility of her being positive is rare as she is tested 10-15 days prior to intercourse.

You have used condom but it is almost like unprotected intercourse.
This act can be considered as minimal risk. The chances hold as for vaginal intercourse.

At this stage you can go for HIV RNA PCR preferably after 10 days of exposure.

You can get back for any clarification.

Dear Dr Bharatesh D Basti

Thank you for replying.

I can not take the tension and stress, I am going insanse. I have a family with very small children. This was a huge mistake and I only realized once the condom broke and i ejaculated inside her. This who episode was 1-3 minutes. She told me she is clean and even her pimp told me she did tests before she came here and she normally gets them done when she is coming as she is one of her regular girls. I am freaking out. There is a clinic in Dubai that does HIV RNA PCR test and they say after (7-10) following exposure. Tomorrow, December 13, 2014 ,will be 8 days post expore and I want to go for test. Will this be sufficient? How accurate are these tests? Should I believe what the girl said about her mdecial status and her pimp? Please help me.

Brief Answer:
HIV RNA PCR test is 95-99% sensitive.

Detailed Answer:
Hi
Thank you for follow up query.

You can trust them if they have repeatedly done testing, as it would affect their business too. I would say it as almost no risk.

PCR test can come positive even after 3 days of exposure. 8 days should be okay. So you can go ahead with HIV RNA PCR testing.

You can at least be > 95- 99% sure of results.

You can get back for any clarification
Thank you
