Question: Hi doctor,
I am a male 31 yrs, had a protected sex with a sex worker in XXXXXXX but had vaginal licking with her. I have an existing gum infection and brushed teeth more than 1 hr before the incident. The doctor prescribed PEP with Truvada 200-300 and Isentress 400. Could you assess my risk and need for completing the course ?

Thanks for posting your query.
A very good combination of PEP.
You had a protected sex, but had a oral sex with gum infection. Oral sex has a low risk for HIV. Gingivitis could slightly increase the risk.
You have to take the PEP for 4 weeks without any interruption without missing any dose. You had not mentioned how long after the exposure you had started these medicines. If it had been started after 72 hours it would not be useful.
If you had started PEP as early as possible within 6 hours the chances of aborting the infection, if any, is absolute.

I have started PEP 17 hrs after exposure.
Also I asked the worker repetitively about her status and she told she is clean according to her last month checkup , doubting the authenticity I regard this as Unknown status.

From the entry of virus into one's body to reach the blood cells (T4 Lymphocytes) it takes not less than 72 hours for HIV. After 72 hours the disease gets established and PEP has no rule the disease is a chronic affair.
You had started PEP by 17 hours. So you are safe. If there would be any infection, it would have been aborted by the completion of PEP.
But as per PEP schedule one should be followed up to 6 months according to the guide lines with regards to periodic blood examination and toxicity monitoring.
