What Are The Chances Of HIV Transmission Through Oral Sex?

Question: Doctor,I had an unprotected Oral sexual encounter wiht a woman of unlown status 85days back.Receptive and giving.I got ANTI HIV1/II-SERUM/CMIA test on 28th/62nd /75th and 83rd day.All Nonreactive(0.32/0.35/0.23/0.29) respectively.Now I understand that these tests are non conclusive and the Window period is 3-6months from the Microbiologist who did these tests.Alternately she suggest I go for HIVDUO or early profile detection test.I consulted your panel earlier who had indicated all is clear after 62nd day test and there was no need to repeat any tests.And I resumed my normal family life.But this talk has put me in cross roads and I am now more worried than before.
I would like to know my actual status and what is the risk I am carrying as I assume I was exposed.Please give you opinion.

Doctor-I had written the name earlier-The report says Test decription:Serolgy"ANTI HIV I/II (SERMU/CMIA)".
Is this a 3rd gen or a 4th Gen test?And how reliable is it on days I took it.
I know the risks are low,but not nil.And my fears are making it very much worse for me.

- If it is only HIV 1 and 2 antibodies test than it is only a 3rd generation and not the 4th generation ELISA. In 4th generation ELISA it tests both P24 antigen and antibodies while in third generation it will only antibodies that can be tested.

- So if it only 3rd generation ELISA it is necessary for you can confirm with early detection profile.

- The probability of HIV in your case of having HIV is very low even though if are feeling afraid you can confirm it with the Gold standard PCR Pro DNA test.

Doctor,Thank you for your reply.I am still not clear for my query:
1-So what I did was a 3rd Gen test?
2-What about my results at 28/62/75/83rd day?Are these totally insignificant and of no use and have no relavance on my HIV status?
3-If I do it at 90days,repeat the same Antibody HIV 1/2 screening test,will that be considered conclusive?if not,what will be taken as conclusive?
4-Why did your earlier expert say all is well after my 62nd day test and no repeat tests are required?Does this mean his reply was incorrect?
Please advice.

1. Yes, you did a 3rd generation test. It only tests antibodies to HIV.

2. It is scenario in over all patients that they are becoming HIV reactive near 90 days after exposure. So samples tested near 90 days (at 83 days) are significant. But it will be more significant once it crosses 90 days cut off.

3. Yes, If you tested it after 90 days it will be more significant.

4. As per NACO (National AIDS Control Organisation) The cut-off time limit for third generation ELISA is 90 days.

Doctor,
Today I did the 90th day test.And the result if Non reactive(0.38).Shall i take this as conclusive?
I was wondering why the values of this non reactive -0.32/0.35/0.23/0.29 and Now 0.38 keep changing and now it is up !!!Is this a cause of worry?
I really have no more strength in me to take more screening tests as the stress and fear is too much.Please comment.
I was reading in lot of journals which say 12 weeks/3months/90days all are the same.Infact the voluntary discloure form which I signed also says that 12 weeks prior activities need to be tested and then it also days 3month should have crossed.So it convieniently makes both12 weeks and 90days as official cut off.

- 90th day HIV non reactive means Confirm HIV non reactive.

- This values of non reactive samples is termed as Optical Density reading(OD), It will keep changing with in normal limit in each reading. It is normal phenomenon & should not be worried.

- 90 days/12 weeks/3 months all should be considered as one cut off point. It is approximately value which counts as 90 days plus or minus 2 to 3 days.
