Question: Hello sir,
I had protected sex with a sex worker in XXXXXXX on 12th feb 2015 but didn;t know whether the condom broke or not but i found my siemens in the reservoir tip while removing the condom..after that i took 2 tests at 2 different labs both are hiv 1/2 and p24 antigen combo tests at 37th day and both are negative.Can you suggest me what to do now? Cause i am really stressed out regularly checking for symptoms now in 7th week i developed 2 canker sores inside my chick and i am loose stools but XXXXXXX 2 times a day but not that loose.Please need help.nd i think i want to know is that the BHIVA,CDC all have updated their testing recommendations but why naco is not updating since 2007?does ars start after 6 weeks?Sir what does jci accredation means?because i did test one of the labs which was a part of jci accredated hospital but i don't remember if the lab tecnician used new syringe or not.so i am getting again paranoid.In XXXXXXX how often this happens in healthcare settings.

Thanks for posting your query.
First of all your exposure is a protected one. As you are able to appreciate semen collection inside the condom while removing ensures that your condom was intact throughout the act. So there is nothing to worry at all.
Regarding the test you had COMBO test (antigen and antibody detection test). Antigen part of the test becomes conclusive by 4 weeks after the risky exposure and the antibody part of the test becomes conclusive by 12 weeks after the exposure. As it was negative by 37 days means that the possibility of HIV 1 infection was completely ruled out (HIV 1 infection prevalence in XXXXXXX is about 99.5% and the prevalence of HIV 2 is less than 0.5%). HIV 2 can be ruled out by 12 weeks by this test. But your chance of getting HIV 2 is remote as you had protected sex.
Canker sores and loose motions can occur to anybody. Without a positive HIV test nobody can predict HIV with these symptoms.
In almost every laboratory in every city in XXXXXXX only disposable syringes are being used. There is no question of the usage of reusable syringes at all.
Avoid your paranoid ideas about HIV. You would not likely to have HIV infection. There is nothing wrong in repeating the test after 12 weeks to rule out HIV 2 infection.

Already it is confirmed that you are free from HIV 1 infection.
Moreover 4th generation COMBO test (P24 Antigen with Antibody 1/2) is more sensitive than the other tests and would be confirmatory a week earlier, so by repeating the same by 70 days you can rule out HIV 2 also by sure.

Thank you very much sir for your detailed reply.Sorry to bother you again.I am having some confusion about diarrhoea.Actually i eat a lot of spicy food and that to in hotels as i am a bachelor.I had this problem earlier also.But i have noticed that when i eat lot of spicy food the next day i am having stools not very loose but quite and XXXXXXX 2 times in that day mostly in day time.But when I eat normal the next day i get stools but not that loose for once a day.Is it related to my exposure as I am facing the same some days.Is this diarrhoea.But I am able to do all my work easily.I am able to eat well and don't feel weak.I regularly check tempereture always 36.8 to 37.1 just one day when I did my test that day before result for 3-4 hours it did go upto 37.6 then back to normal.Also I forgot to mention that after 2 days of blood test I got a light red patch near elbow slightly big not raised but under the foreskin.I thought it to be side effect of drawing blood near elbow not at the same point nearer to it.it went in one week but gradually fading its colour day by day.

With protected sex with intact condom, you are unlikely to have HIV. Passing loose motions then and there is quite common with those eating in Hotels. Your elbow lesion also was not related to HIV. Just initiate your physician to check the motion and you can have a course of treatment, if there is a need. Otherwise just ignore about HIV infection.

Thanx a lot.Sir this is the last question.Actually last some days when make neck full down i am able to feel lymph nodes under jaw moving when touched and not painful but when i make my neck straight i am not able to fill anything and i am unable feel my lymphnodes anywhere.And I am from electrical field so before 2 week when working in field i was accidentally poked by an armour of a cable and after just some seconds little blood come out of finger.The armour was open since many long time.So I am again tensed.from last two days i have developed a red itchy bump on right bicep and one non itchy bump on left elbow.What might it be??

Lymph node enlargement over neck can occur to anybody due to various reasons. Only a thorough examination and appropriate investigations after the right time could only either confirm or rule out the diagnosis of any condition.
Similarly itchy bump and non itchy bump over your right arm and left elbow also have to be examined by a Dermatologist to interpret any possible diagnosis. Even it could be due to an insect bite allergy. Presumptive diagnosis can not be made only with your words. It would go wrong. All these things not necessarily be due to HIV.

Hello sir,
I found out that the phadke labs mumbai(SRL diagnostics) do HIV-2 RNA QULITATIVE PCR.Shall i go for the test now as i am now on my 63rd day of the exposure to rule out HIV-2 or shall i wait for some time to test for HIV1/2 and P24 antigen combo test.I am really stressed about the symptoms.But the 1st test is somewhat costly.So kindly suggest whether there will be false positive cases in RNA PCR?As I have already done 2 combo tests on 37th day post exposure.

You need not wait till 90 days. When HIV 2 RNA PCR Qualitative test is available it would be confirmatory and conclusive still earlier around 4 weeks on wards. That would also be definitely non reactive for HIV 2. You can go for the same without any hesitancy or doubt.

Thanx sir.I have attached a photograph of the previously discussed slightly itchy bump on the right arm.Doe it look like ars rash?And sir based on my previous tests shall i again have to do test for HIV-1?Based on my type of exposure what are my chances being infected?

With a rash alone nobody can suspect HIV or AIDS related Syndrome or acute infection. Only a reactive blood test can confirm the disease. The rash has nothing specific for HIV or acute HIV infection.
You need not go for further test for HIV 1 infection. It is conclusive that you are HIV 1 Free.
Chance of you being infected with this protected exposure is quite unlikely.
